Great North Medical Group is seeking a passionate and highly committed Advanced Practitioner (Nurse) (ANP) to become a key member of our established and supportive nursing team. Main duties of the job

As an integral member of the clinical team, the successful candidate will work autonomously using advanced clinical skills to assess, diagnose and manage patients with a wide range of health needs. You will make informed clinical decisions, including interpreting diagnostic tests and investigations.

The role includes independently prescribing treatments where qualified and referring patients to other healthcare professionals or specialist services when required. Care will be delivered through face-to-face consultations, telephone assessments and home visits where appropriate.

Working within your professional scope of practice, you will provide expert advice to patients, carers and colleagues, maintaining high standards of clinical care and patient safety.

Key responsibilities include triaging and prioritising patient needs, arranging investigations, making timely referrals and managing a defined patient caseload. You will coordinate patient care, including treatment planning, referrals, hospital admissions and discharges.

As the organisation operates under an annual service contract, the duties and focus of this role may change in line with service requirements. The post holder will therefore be expected to demonstrate flexibility in supporting service delivery as responsibilities may vary from year to year.

Clinical Nursing Practice

The Advanced Nurse Practitioner (ANP) will deliver high-quality, patient-centred care within the practice by:

  • Working collaboratively within a multi-disciplinary team to support the health and wellbeing of a diverse patient population.

  • Acting as the first point of contact for patients with undifferentiated conditions; undertaking clinical assessment, diagnosis and developing management plans.

  • Managing acute and chronic conditions using evidence-based pharmacological and non-pharmacological treatments tailored to individual needs.
