Great North Medical Group is seeking a passionate and highly
committed Advanced Practitioner (Nurse) (ANP) to become a key member of our
established and supportive nursing team. This role offers the opportunity to

Main duties of the job
As an integral part of our team, the successful candidate
will practice autonomously, utilising their professional expertise to make
informed decisions based on assessments, diagnoses, and interpretation of test
results. You will independently prescribe appropriate medications, evaluate,
and where necessary, refer patients to other specialists. Your responsibilities
will include providing care through face-to-face appointments, telephone
consultations, and home visits.
Working within your professional scope of practice and
agreed limits, you will offer expert clinical advice to patients, carers, and
colleagues, ensuring that clinical safety and excellence are always maintained.
Key responsibilities also include prioritising and triaging
patient needs, making timely referrals for further investigations, and
effectively managing your patient caseload. You will be accountable for
overseeing patient care, including treatment, referrals, admissions, and
discharges.

Clinical Nursing Practice The Advanced Nurse Practitioner (ANP) will deliver high-quality, patient-centred care within the practice by:
- Working collaboratively within a multi-disciplinary team to support the health and well-being of a diverse patient population.
- Serving as the first point of contact for patients with undifferentiated conditions; conducting thorough assessments, diagnosing, and developing management plans.
- Managing acute and chronic conditions using evidence-based drug and non-drug treatments tailored to individual needs.
