Job Description Nurse Practitioner
Responsible to Senior Partner - clinically and Business Manager

Job Summary
The post-holder
will be competent in the delivery of primary care to patients in the practice
population (with the only exception being children under the age of one),
having direct clinical involvement and extensive clinical knowledge.
To assess, diagnose, treat, refer or signpost
patients who attend surgery with undifferentiated or undiagnosed conditions.
They will work at
an independent and autonomous Care Practitioner level providing a first point
of access in primary health care
To undertake home visits, including
Nursing/Residential Homes primarily for acute conditions, as assessed by your
duty doctor, and within the boundaries of professional qualifications.
The post holder will use advanced clinical
skills to provide education to patients, promoting self-care and empower them
to make informed choices about their treatment.
They will be able
to provide clinical supervision and support to the nursing team, advice and
support while liaising with other practice staff.
To enable
clinicians to safely travel to visit patients in their own home; they are
obliged to ensure that your vehicle is in good working order and covered by
business class car insurance (any additional premium will be paid by the
practice upon receipt of a copy of the vehicle insurance cost)
Upon
request, a percentage of the Nurse Practitioners time may be spent on personal
and practice development activities including audit, research, team development
and clinical governance.
Job
Responsibilities:
Diagnosing and treating patients
Triage and treat patients
wishing to see a health care professional, making any necessary referrals to
other members of the primary health care team
Acute care providing clinical support/dealing with patients booked
into the GP On Call clinics
Domiciliary Care seeing patients in their own homes who are too ill to
attend surgery and/or performing a ward round/virtual ward round at our
registered Nursing Homes
Patients with problems needing referral to
secondary care should be discussed with the Usual GP before making such
referral.
Ensure clinical Practice is safe
and effective and remains within boundaries of competence, and to acknowledged
limitations.
Advise patients on general
health care and minor ailments, with referral to other members of the primary
and secondary health care team as necessary
To undertake home visits,
primarily for acute conditions, as assessed by the Duty Team, and within the
boundaries of professional qualifications.
To communicate at all levels
within the team ensuring an effective service is delivered.
To maintain accurate, up-to-date healthcare
records appropriate to the consultation.
Ensure evidenced-based care is
delivered to the highest standards.Enhance own performance through
Continuous Professional Development, imparting own knowledge and behaviours to
meet the needs of the service.
To participate in the audit
process, evaluation and implementing plans and Practice change in order to meet
patient need.
Contribute positively to the
effectiveness and efficiency of the team and work colleagues.
Pathological specimens and investigatory procedures
Undertake the collection of pathological
specimens including intravenous blood samples, swabs etc. Perform investigatory
procedures requested by the GPs
Administration and professional responsibilities
Participate in the
administrative and professional responsibilities of the Practice team
Ensure SystmOne is kept up to
date, with accurate details recorded and amended
Ensure accurate completion of
all necessary documentation associated with patient health care and
registration with the Practice
Ensure collection and
maintenance of statistical information required for regular and ad hoc reports
and audit
Attend and participate in Practice
meetings as required
Training and personal development
Training requirements will be
monitored by yearly appraisal and will be in accordance with Practice
requirements. Personal development will be encouraged and supported by the Practice.
It is the individuals responsibility to remain up to date with recent
developments.
Participate in the education and
training of students of all disciplines and the induction of all members of the
Practice staff where appropriate
Maintain continued education by
attendance at courses and study days as deemed useful or necessary for
professional development.If it is necessary to expand the
role to include additional responsibilities, full training will be given.
The post-holder will participate
in any training programme implemented by the Practice as part of this employment.
Taking responsibility for own
development, learning and performance and demonstrating skills and activities
to others who are undertaking similar work.
Liaison
As well as the nursing team,
there is a need to work closely with Administration to ensure the smooth
running of the Practice, reporting any problems encountered to the relevant
person.
There is also the need to
establish and maintain good liaison with other surgeries and agencies,
including secondary care.

In the course of seeking
treatment, patients entrust us with, or allow us to gather, sensitive
information in relation to their health and other matters. They do so in confidence and have the right
to expect that staff will respect their privacy and act appropriately
In the performance of the duties
outlined in this job description, the post-holder may have access to
confidential information relating to patients and their carers, Practice staff
and other healthcare workers. They may
also have access to information relating to the Practice as a business
organisation. All such information from any source is to be regarded as
strictly confidential
Information relating to
patients, carers, colleagues, other healthcare workers or the business of the Practice
may only be divulged to authorised persons in accordance with the Practice
policies and procedures relating to confidentiality and the protection of
personal and sensitive data
Communication
The post-holder should recognise the importance of effective
communication within the team and with patients and will strive to recognise
peoples needs for alternative methods of communication and respond
accordingly.
Health & Safety
Practice staff will comply with all policies in
relation to Health and Safety and ensure they complete any mandatory training
as advised by the Practice Manager.Maintain an up to date knowledge of health and
safety and infection prevention & control statutory and Practice
guidelines.

workplace according to Practice guidelines
Identifying the risks involved in work
activities and undertaking such activities in a way that manages those risks
Making effective use of training to update
knowledge and skills
Using appropriate infection prevention &
control procedures, maintaining work areas in a tidy and safe way and free from

Information
Governance
Practice
staff must keep up to date with the requirements of information governance and
must follow Practice policies and procedures to ensure information is dealt
with legally, securely, efficiently and effectively. Staff must appropriately
manage the records they create or hold during the course of their employment
with the Practice, making the records available for sharing in a controlled
manner, subject to the statutory requirements and agreed security and
confidentiality policies, procedures and guidelines (e.g Freedom of Information
Act 2000, Caldicott guidelines). If you are required to make entries into
health records, you must ensure these entries are legible and attributable, and
that record keeping is up-to-date.
