Job summary

JOB RESPONSIBILITIES

  • See patients that are acutely unwell.
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.
  • request pathology services as necessary.
  • Process and interpret pathology and other results that you have requested.
  • Recognise when patients need to be referred to secondary care and complete the referral.
  • Action any A&E documents or out of hours documents. be part of the duty doctor rota.

Main duties of the job

  • Manage patients presenting with a range of acute medical conditions.
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Prioritise health issues and intervene appropriately
  • Recognise, assess and refer patients presenting with mental health needs
  • Extended and supplementary prescriber, adhering to extant guidance
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
  • Contribute to practice targets (QOF etc.), complying with local and regional guidance
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies

We are looking to recruit an full time Advanced Nurse Practitioner to work within our existing teams to deliver services to patients with minor illness or long term conditions. You will be working alongside Clinical Practitioners, Nurses and a well supportive team.

Our premises are modern and purpose built and we have a branch site on Blacktstock Road, S14. Our team consists of 4 Partners, 5 Salaried, 2 Advanced Clinical Practitioners, 3 Practice Nurses, a Nurse Associate, 2 HCAs, and Phlebotomist. We have a well-established management team, brilliant admin team and a training practice for Registrars, Medical Students and VTS nurses.

We believe in Person-Centred, holistic care, working in partnership with our patients and local community. We believe in social prescribing and have a tailored long-term condition service. We pride ourselves in being at the cutting edge of education, innovation and service delivery. We have a strong focus on high quality patient care and working as a team.

We support the green agenda, believe in low carbon transport and personal health & well-being. We hold weekly team meetings which include MDT, SEA and educational sessions.

We will be joining a Peak Edge Primary Care Network in April 2024 and are looking forward to be working with likeminded practices who share our vision. We pride ourselves on our commitment to our patients, delivering care to enable them to optimise their health and well-being via our multi-disciplinary team approach. We are now in a position to recruit our PCN staff.

This post will be providing care and treatment to Sloan Medical Centre patients only and based at the medical centre or branch site.

We work hard as a team to support our patients but are always mindful to support each other. Building a strong relationship within our team also builds a strong support structure.

We aim to deliver enhanced services to support all our patients needs and aim to increase the health and well-being of our population. To do this we will employ staff and find innovative and sustainable solutions to the changing needs of patients in a modern NHS.

We are looking to recruit an Part Time Advanced Nurse Practitioner to work within our existing teams to deliver services to patients with minor illness or long term conditions. You will be working alongside Clinical Practitioners, Nurses and a well supportive team.

We can offer the successful candidate the following;

  • Salary £25p.h - £27p.h (pro-rata depending on experience)
  • Hours: 37.5 hours per week, over 5 days
  • NHS pension
  • Holidays: 6 weeks plus one week study, pro-rata
  • Regular training and supervision
  • Weekly MDTs
