JOB RESPONSIBILITIES
- See patients that are acutely unwell.
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.
- request pathology services as necessary.
- Process and interpret pathology and other results that you have requested.
- Recognise when patients need to be referred to secondary care and complete the referral.
- Action any A&E documents or out of hours documents. be part of the duty doctor rota.
Main duties of the job
- Manage patients presenting with a range of acute medical conditions.
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Maintain accurate clinical records in conjunction with extant legislation.
- Prioritise health issues and intervene appropriately
- Recognise, assess and refer patients presenting with mental health needs
- Extended and supplementary prescriber, adhering to extant guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF etc.), complying with local and regional guidance
- Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
- Support the clinical team with all safeguarding matters, in accordance with local and national policies

Our premises are modern and purpose built and we have a branch site on Blacktstock Road, S14. Our team consists of 4 Partners, 5 Salaried, 2 Advanced Clinical Practitioners, 3 Practice Nurses, a Nurse Associate, 2 HCAs, and Phlebotomist. We have a well established management team, brilliant admin team and a training practice for Registrars, Medical Students and VTS nurses.

This post will be providing care and treatment to Sloan Medical Centre patients only.

We are looking to recruit an full time Advanced Nurse Practitioner to work within our existing teams to deliver services to patients with minor illness or long term conditions. You will be working alongside Clinical Practitioners, Nurses and a well supportive team.
