Job summary

We are looking for a dedicated and proactive Paramedic, who is passionate about high-quality patient care to join our friendly multi-disciplinary team.

As part of the Purley & Coulsdon PCN, you will work closely with our GPs, nurses, ANPs, physiotherapists and administrative teams to deliver safe, effective, and person-centred care across our population of just under 18,000 patients.

We are a supportive practice and take pride in helping clinicians develop, thrive and build long-term careers in general practice.

This post is a part-time role for 20 hours per week.

Main duties of the job

As a Paramedic, you will be a valued member of the clinical team, responsible for:

Clinical Work

  • Telephone and face-to-face consultations for minor illness/injury
  • Clinical assessment, diagnosis, treatment and referral
  • Community and care-home visits
  • Advising patients on health and wellbeing
  • Maintaining accurate clinical records
  • Supporting audits and service improvements

Operational & Collaborative Work

  • Working within own professional scope and code of conduct (HCPC and College of Paramedics)
  • Supporting PCN quality and outcomes framework (QOF) and other targets
  • Contributing to audits, service improvement, and implementation of local and national guidelines

Job description

Job responsibilities

- Work as part of a multi-disciplinary team (MDT)

- Assess and triage patients, including same day triage and as appropriate, provide definitive treatment, following patient group directives, NICE (national), local clinical guidelines and local care pathways and make necessary referrals to other members of the primary care team

- Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the organisations social prescribing service and, where appropriate, other community or voluntary services

Be able to -

Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance

Perform and interpret ECGs

Perform investigatory procedures as required and undertake the collection of pathological specimens including intravenous blood samples, swabs and other samples within their scope of practice and within line of local and national guidance

Support the delivery of anticipatory care plans and lead certain services (e.g. learning disability health checks, SMIs and diabetic reviews of care home and housebound patients)

Provide an alternative model to urgent and same day GP home visit for the practice

Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered

Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the organisation

Communicate effectively with patients and, where appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices
