Job summary
The ideal candidate should
have a good understanding of Primary Care. a knowledge of Chronic Disease Management, Immunisation and Cytology experience or be
willing to undertake any necessary training required for the role. They should have an understanding of Qof and
be committed to providing excellent care to our patient population.
Main duties of the job
To work as a team with the Partners, Practice Nurse and administration
staff to provide skilled and effective care to the practice population
To deliver care in accordance with the agreed Practice protocols and
objectives
To assess, plan, deliver and evaluate care holistically to the practice
population
Delivery of Chronic Disease management
Clinical Support on the management and care of patients with Long Term
Conditions
Plan, implement and evaluate individual treatment plans, within the
remit of your own role

We are a 3 Partner, 3 Salaried GP, 3 ANP and 3 PN team supported by 2 HCAs and 1 TNA and a full Management and Admin team. We are situated next door to Sunderland Hospital in a purpose built building. We work together to provide the best service possible to our 10,300 patients from across the city.
