Job summary

This post is to replace a member of staff who is retiring. The Nile Practice is a 2 Partner Practice with over 10K patients across 3 sites. We are looking to recruit a part time receptionist/care-navigator. The normal days of work will be Thursday and Friday, but flexibility is required to cover holidays and sickness.

Main duties of the job

To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practices multidisciplinary team. The receptionist/care navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.

About us

We are a teaching practice and have 2 GP partners, Locum GP's, Pharmacist, registrars and medical students. They are supported by 2 ANP's, 3 Practice Nurses and 3 HCA's. The Practice has 3 sites - Cheslyn Hay, Cannock and Hednesford.

Job description

Job responsibilities

The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:

a. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition

b. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately

c. Process patient requests for appointments

d. Process repeat prescription requests

e. Initiate contact with and respond to requests from patients, team members and external agencies

f. Enter read-code data on EMIS

g. Photocopy documentation as required

h. Data entry of new and temporary registrations and relevant patient information as required

i. Input data into the patients healthcare records as necessary

j. Process referrals using electronic referral system (ERS)

k. Direct requests for information, e.g. SAR, insurance/solicitors letters and DVLA forms, to the appropriate team member

l. Manage all queries as necessary in an efficient manner

m. Carry out system searches as requested

n. Maintain a clean, tidy, effective working area at all times

o. Monitor and maintain the reception area and noticeboards

p. Support all clinical staff with general tasks as requested
