Main duties of the job
The successful candidate will work as part of a multidisciplinary team to provide high-quality, evidence-based care for patients with diabetes. This role involves leading and supporting diabetic clinics, offering education and management plans to patients, and contributing to the development of diabetes care within the practice.

Job responsibilities
- Deliver specialist nursing care, support and advice to patients with Type 1 and Type 2 diabetes.
- Conduct diabetes reviews, including blood glucose monitoring, insulin management and medication.
- Support patients with lifestyle education, including diet, exercise, smoking cessation and self-management.
- Manage reviews as part of the QOF framework
- Work closely with GP's, pharmacists, practice nurses, healthcare assistants and external specialists to optimise patients care.
- Contribute to the development and implementation of diabetes management protocols and clinical pathways.
- Maintain accurate, up to date clinical records in line with practice and NMC standards.
- Participate in clinical audits, meetings and ongoing quality improvement initiatives
- Provide input into patients recall systems and ensure follow-up of tests results and referrals.
