An opportunity has arisen for an Advanced Clinical Practitioner / Advanced Nurse Practitioner to join our friendly and supportive Practice team.
We are looking to appoint a motivated, enthusiastic and experienced Clinical Practitioner who is able to work effectively within our clinical team to undertake patient assessment, diagnosis and clinical management.
Home visits are a requirement of this role.

Main duties of the job
Working as an independent prescriber who will be able to diagnose and treat patients of all ages presenting with acute problems and chronic diseases, via face to face, video and telephone consultations.
The candidate will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The role requires cross site working at both locations.
About us
We have 2 sites, 1 in Cleckheaton with around 7,000 pts and the 2nd is in Ravensthorpe and we have around 4,600 pts there. The cohort of patients are different at each site.
At each site there is a team of reception / admin staff who support the clinicians with the day to day delivery of services to our patients.
