Main duties of the job
The pharmacy Technician will be part of the
Medicines Team responsible for a combination of patient care.
The successful candidate will be supported in terms
of learning and professional development through regular access to GPs,
clinical pharmacists and through a formal education pathway through the Centre
of Postgraduate Pharmacy Education. As part of a growing team of there is also
a programme of peer support events and team meetings in place.
About us
Our PCN comprises of our own group of 5 practices across the Chorley area.
These practices form a large and dynamic network, with a proven track record of
successful and innovative projects. Our larger size allows for more
collaborative working and due to the success of integrating the role into our
practices, we are looking to grow our team and recruit additional Registered
Pharmacy Technician to work alongside the current team.

Job responsibilities
- Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting.
- Support colleagues and patients with medication queries and monitoring.
- Undertake non-complex medication reviews.
- Using audits and action planning to help to tackle inequalities.
- Provide person-centred medicines optimisation.
- Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services.
- Work with general practice teams to ensure that practices are compliant with CQC standards where medicines are involved.
- Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system
- Take a role in aspects of shared care protocols, with medicines, liaison with specialist pharmacists, liaison with community pharmacists.
