Job summary

Care Coordinators works with both patients and Healthcare Professionals to ensure seamless, organised and effective care. Coordinators liaise with patients to monitor the care provided and work with the care team to assess the treatment.

Main duties of the job

The primary role of a Care Coordinator is to advocate for patients to make sure they get high-quality care. They perform a range of administrative tasks, such as entering patient data, answering phone calls, organising MDT meetings and scheduling patient visits. The role involves a significant amount of communication with patients, their family members and various Healthcare Professionals. The Care Coordinator synthesises information from these people, such as patient needs and healthcare goals, in a way thats useful and helpful to the patient and Healthcare Professionals.

Our larger size allows for more collaborative working and due to the success of integrating the role into our practices, we are looking to grow our team and recruit additional PCN Care Coordinator.

Chorley and South Ribble PCN is made up of the following practices:

Job description

Job responsibilities

Build strong and collaborative relationships with local community and voluntary organisations, seeking new opportunities.

Run audits and searches where necessary to identify patients for review.

Serve as the point of contact for practice staff, providers, care teams, family/caregivers and community resources where indicated, responding with empathy and respect.

Help people manage their care needs by answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information relating to their care.

Support the PCN to deliver and report on quality metrics, such as QOF, KPIs and locally commissioned enhanced services by documenting and monitoring aspects of patient coordination and service delivery,

Attending multi-disciplinary team meetings working as part of the wider holistic team to provide support, as necessary.

Liaise with all clinical and non-clinical members in the MDT to ensure effective MDT function.

Build and maintain relationships with members of the local support team including named GPs, pharmacists, community nursing teams, therapists, dementia nurses etc.

Regularly attend the PCN meetings to update the wider team on your work.
