Job summary

We are looking to recruit an enthusiastic, reliable and highly motived QOF GP Assistant to join our existing team of over 70 staff looking after circa 24,500 patients.We are looking for 2 part time roles which will be part of a job share.

The position is based at Lostock Village Surgery 3 days a week, however there may be a need to work at our other Practice locations when required.

A valid driving licence is essential for the post.

Main duties of the job

You will work alongside the organisations Care Coordinators and Population Health Manager to manage our Chronic Disease Register and achieve all QOF targets annually.

About us

We comprise of 6 surgeries with our own Primary Care Network Status; Buckshaw Village Surgery, Eaves Lane Surgery, Adlington Medical Centre, Croston Medical Centre, Lostock Village Surgery and Medicare Unit Surgery. Our total patient list size is combined at 23,000 and is growing rapidly. We work closely with neighbouring practices as part of a collaborative group to develop innovative primary care service to patient population in excess of 30,000.

We currently employ a total of 70 members of staff; we take pride in working together to deliver the best care for our patients.

The practice is constantly evolving to take advantage of the opportunities arising in the ever-changing NHS, and to find new and better ways of working.

Job responsibilities

The role will see the successful candidate:

Coordinate care for disease prevention.

Have a good understanding of QOF targets and ensure patients are called for review in a timely manner.

Regularly search for patients with poor disease control and target them for additional support.

Actively search for patients with high BMI or smokers to support to change.

Refer/signpost patients to smoking cessation, exercise, and weight loss services.

Ensure Practice Nurse clinics are used efficiently and effectively by actively managing clinic bookings and patient recall.

Ensure patients have the correct preparation for their appointment and are followed up if they DNA by means of a telephone call to encourage patients to attend and improve their understanding and realise the importance of Chronic Disease reviews.

Ensure early follow up for any Chronic Disease review if needed more frequently than annually.

Ensure patients have the most appropriate appointments if they have co-morbidities to improve engagement and compliance.

Work with Partner Products such as iPlato to invite patients digitally and ensure we hold up-to-date patient information to enable us to target a wider population digitally.

Build trust and a positive dialogue with patients who face challenges with their health.

Have a flexible approach and a focus on what matters to patients.
