Job summary

We are looking for a forward thinking, motivated Clinical Pharmacist to join our expanding Primary Care Network team working for the Central Slough Network (CSN) based at Crosby House Surgery Primary Care Network (PCN).

The successful candidate will be based at Crosby House Surgery. They will be an independent prescriber and will be closely supported by the PCN Clinical Director, GPs and PCN Lead Clinical Pharmacist. Though employed by Crosby House Surgery the successful candidate will work as part of the CSN PCN Team.

Main duties of the job

  • The individual in this role will work within a multidisciplinary team, engaging directly with patients to evaluate and address their health issues and encourage self-management.
  • Undertake chronic disease reviews for patients with multi-morbidity, implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
  • Evaluate the ongoing necessity for each medication, assess monitoring requirements, and offer assistance to patients in managing their medication to ensure optimal use (i.e., medication optimisation).
  • Ensure patients receive the best possible outcomes from every interaction. Foster and maintain strong links to additional services across the locality for potential future collaborative working.

About us

Crosby House Surgery is a GP Training Practice based in Slough. We are a 2 partner practice who currently employs 5 salaried GP's, 2 pharmacists, 2 first contact physiotherapists, a physicians associate, 3 registered nurses, 1 HCA and an experienced admin and reception team to support the clinicians. We have a list size of 12,000 patients. We are high QOF achievers and a paper lite surgery, utilising the latest technology to enhance patient care.

Job responsibilities

  • To undertake medication review clinics with people with multiple conditions and on polypharmacy. Supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues.
  • To support patients discharged from the hospital by ensuring they understand their medication, any changes that have happened recently and have a timely supply.
  • Ensure that the practice repeat prescribing policy is regularly reviewed and that it is being adhered to with the aim of reducing waste and improving safety.
  • To help plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing.
  • Working closely with GPs to resolve day-to-day issues related to the medication.
  • To act as a point of contact in the practice for all medication and prescription-related queries from patients and health care professionals.
  • To undertake minor ailment clinics or triage.
  • To undertake medication audits. To manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions.
  • Participate and deliver education and training to practice staff of medication-related topics.
  • To produce patient information leaflets and posters and run medicine awareness projects throughout the year.
  • To undertake clinical audits to support the practice in following NICE and local guidance and pathways.
  • To take responsibility for your own development and attend training events as appropriate.
