Job summary
We are seeking a Practice Nurse essentially to join our friendly practice, but also somebody who will be committed to supporting the whole practice team effort.
This role primarily will provide the medical care and support to our LTC patients. We are looking for somebody who is motivated as well as being able to communicate and understand the needs of the patient.
In addition, we are looking for somebody who will give commitment to ongoing professional development and who also has high levels of integrity and loyalty.

The main duties of the role are as follows:-
Primarily looking after the health and wellbeing of our diabetic patients.
Childhood immunisations.
Asthma reviews.
COPD reviews.
Cervical cytology.
Travel vaccinations.
LTC reviews.

We are a high earning PMS training practice based in Tyldesley, Greater Manchester, within easy access of the town centre and local bus routes. We are a friendly, dedicated practice with a determination to provide high quality clinical care for our patients, at the same time we are highly committed to looking after our Doctors too. We have a list size of 5000 patients, with a full compliment of support staff consisting of 2 GP partners, 1 ACP (in training) , 1 Practice Manager, 1 HCA, 6 receptionists. We have a practice pharmacist, access to First Contact Practitioners, Social Prescribers and Care Co-Ordinator's via our PCN ARRS roles. We provide a wide range of facilities in house, such as minor surgery, joint injections, private medicals. We use EMIS Web clinical system and AskMyGP online consultation service.

Request pathology services as necessary and process pathology results as required.
Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.

Maintain accurate clinical records and to ensure read codes are used effectively.
Maintain chronic disease registers.
Develop, implement and embed health promotion and well-being programmes.
Implement and evaluate individual treatment plans for chronic disease patients.
Support the team in dealing with clinical emergencies.
Manage Emergency Drug Kit.
Monitor and ensure the safe storage, rotation and disposal of medicines.
Support and participate in shared learning within the practice.
Continually review clinical practices, responding to national policies and initiatives where appropriate.
