Advanced Nurse Practitioner

An exciting and challenging opportunity has arisen for an Advanced Nurse Practitioner to join our friendly practice team. This position will cover our enhanced access sessions (every Thurs 6pm - 8pm and every 3rd or 4th Saturday) and planned leave cover for other ANP colleagues.

We are looking for a dynamic ANP with excellent communication skills to undertake a variety of duties including same day face to face consultations, telephone triage, e-consults and home visits.

You will be experienced, dynamic and committed to providing high standards of care. You will demonstrate safe clinical decision making working collaboratively with the general practice team to meet the needs of our patients.

Experience working in a general practice environment is essential.

Main duties of the job

  • Working independently and alongside GPs to identify appropriate care paths and treatment methods for patients presenting with acute problems, through face to face appointments, telephone consultations and home visits
  • Making professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other health care professionals
  • Using clinical nursing practice at an advanced level to deliver quality care and advice to patients and demonstrate an in depth knowledge of the options for achieving control of symptoms and assessing the health needs of patients with undifferentiated and undiagnosed problems
  • Perform safe triage of patients when necessary via telephone, video, e-consultation and face to face consultations
  • Prescribe medication appropriate to patient needs or refer the patient to a community pharmacy who will provide designated minor aliment services and prescription advice
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Support and educate patients to adopt and promote healthy lifestyles and apply principles of self-care

Skerne Medical Group is a rural practice registered with the Care Quality Commission. The practice provides services to around 16,750 patients from three locations: Harbinson House in Sedgefield, Carroll House in Trimdon Colliery and Beveridge House in Fishburn.

We area well-established training practice providing high standards of patient focused care. Our practice team consists of 8 GP Partners, 3 Salaried GPs and3 Clinical Practitionerswho are supported by an extensive experienced nursing team along with a full complement of management and administrative staff who all play a part in striving to provide the best possible healthcarefor our patients.
