Vida Healthcare are looking for a compassionate and hardworking person to join
our Community Support team, formally known as our ABC team.
The ABC team provide
fantastic community support focussing on the elderly population, dementia and
frail. It is a very rewarding job giving
you the opportunity to make someone's life better or support them when they
need it the most.
The role requires someone
who is dynamic, compassionate and vigilant.
The role covers the social and clinical elements and communication is
key.
It involves
networking and working with other clinical and social professionals. This role will include clinics at the surgery and home visits; thus,
travel is essential. You will be responsible for your own mode of transport for
this role.
Main duties of the job
These are patients who are housebound, and who can't
attend the surgery, plus the huge amount in the middle who are really
struggling. These patients need help and support to maintain their independence
safely at home. The Community Support Team are vital
communication for these patients and their families.
The main centre of the role is memory assessments; dementia
reviews and working with GPs to ensure patients and their family have access to
both social and medical support.

Qualifications
Essential
- Educated to GCSE level, with English being one of those GCSE passed, or two years working experience.
- Previous computer training and experience
- Ability to work effectively with others.
- Excellent organisational, time management, communication skills, and attention to detail.
- Ability to work with highly confidential information.
Desirable
- Previous social training background or healthcare assistant background
- Previous experience using SystmOne
- Entry level knowledge of emails and microsoft products.
- Previous experience of working within the NHS
