Job summary
Exeter City PCN is looking to recruit a PCN pharmacist to our well established Pharmacy Team. Our pharmacists support patients with medication queries, including repeat
prescribing authorisations, undertaking clinical medication reviews either face to face or via telephone as well as advice for homely remedies and over the counter medications.
You will be an integral part of the team, bringing a wealth of knowledge and skill, working as part of both our duty and routine care teams.
Main duties of the job
Main Duties
Help achieve efficiencies and effectiveness within the prescribing activities of the
PCN group of GP practices including supporting a 'prescription hub'.
Promote high quality cost-effective and evidence-based use of medicines within the
PCN
Work within the Pharmacy Team, with other clinical pharmacists to respond to
medication-related queries generated by patients, GPs, other health care
professionals and staff
Work within the Pharmacy Team, with other clinical pharmacists to develop and
ensure a safe efficient medicines optimisation programme including repeat
prescribing, medication review and medicines reconciliation services
Undertake clinical medication reviews either face to face or via telephone support
on identified stratified-risk patients that may include residents in care homes, in
their own, with long-term conditions, and/or learning disabilities
Identify and produce recommendations for medication optimisation for senior
clinical pharmacists, GPs and other multi-disciplinary team members
Work with care home staff to improve safety of medicines ordering and
administration if required
Work to encourage good management of common/minor/self-limiting ailments,
including promotion of self-care and homely remedies
Contribute to the safety of drug monitoring processes and drug alerts
Analyse, interpret and present medicines data to highlight issues and risks to support
decision making
Advocate and implement formulary recommendations.
About us
Exeter City PCN is a well established partnership of practices based within Exeter. The team consists of clinical pharmacists, pharmacists, MH Nurses, Physiotherapists, Wellbeing Coaches, Cancer Care Support workers and a dedicated care home team.
The PCN have established GP leads supporting allied health care staff across the PCN with defined career pathways. The PCN values individuals promoting the CPPE course for pharmacists and developing relationships across networks to maximise opportunities for professional growth and service development.

The post holder will work within their clinical competencies as part of a multi-disciplinary
team to provide expertise in clinical medicines management, provide face to face structured
medication reviews, manage long term conditions, management of medicines on transfer of
care and systems for safer prescribing, manage repeat prescription authorisations
processes, while addressing both the public health and social care needs of patients in the
GP practices that make up the PCN.
The post holder will provide leadership on quality improvement and clinical audit and well
as managing some aspects of the Quality and Outcomes Framework. This role is pivotal to
improving the quality of care and operational efficiencies so requires motivation and
passion to deliver an excellent service within general practice.
As well, the post holder will develop and encourage a positive culture of medicines
optimisation principles to assist the PCN in achieving local and national medicines-related
objectives.
