We are recruiting a Mental Health OT or Clinical
Psychologist
Come and join the largest primary care Mental Health and Wellbeing team in Exeter!
As a PCN we are currently working towards some exciting new
projects to forge ahead with transformation in same day care, putting patients
at the centre of everything we do.
As part of this transformative approach to care we are
looking to recruit a MH OT or Clinical Psychologist to support and develop the
team working as part of a larger multi-disciplinary team. The role will support
existing services and also help develop a new and exciting pathway to supporting
low risk patients to access care and services available on the day.
This would be an excellent next step for anyone looking to
develop their skills within primary care, looking to step into a lead role.
Main duties of the job
You will manage a caseload of clients with complex needs,
using evidence based/client centred principles to triage, assess, plan, implement and
evaluate interventions. You will provide leadership for students and/or
junior staff, through supervision and appraisal. You will participate in the
planning, development and evaluation of MH and wellbeing services and work with
Health and Wellbeing Coaches, Social Prescribers and other professionals as
part of a wider MDT team.
In addition you will help to design and inform the same day
service team and the contribution that can be made by the PCN MH and Wellbeing
Team, working alongside clinical pharmacists and physiotherapist to provide a
truly integrated Multi disciplinary team to provide same day support for
patients.
About us
Exeter City PCN support 36000 patients across 6 practices. We already have a well-established Mental Health and Wellbeing Team who work in partnership with our primary care clinicians to provide more services and support closer to patients. Sadly our brilliant MH OT lead is moving on to other opportunities, so we are looking to appoint a proactive MH OT or Clinical Psychologist to take on the role.
The team consists of 2 health and wellbeing coaches, a MH nurse, a young persons MH support worker and a brief assessment and intervention worker. Over the past three years the team has developed and grown into the role of providing interventions, coaching and support to primary care patients. They have been very successful in building strong links with Voluntary and Charitable organisations making excellent use of community contacts to support patients.
Their innovative approach has help shape and develop services across the PCN such as our Womens Menopause Service, Healthy Movement and Young Peoples Services to engage with support. The team are brimming with ideas and we want to help them achieve them.

To oversee a small team comprising
of a Health and Wellbeing Coaches and social prescriber working together to support and activate patients presenting with
mild or moderate mental health
To set up processes to enable timely review
and appropriate signposting to make best use of the available capacity of the
team.
To select and apply appropriate assessments for a designated caseload, addressing enabling the client in areas of self-maintenance, productivity
and leisure.
To work with service users to identify goals as part of the overall care plan working within the
care programme approach process.
To plan and implement individual and/or group
interventions, in collaboration with the service users, to achieve therapy goals.
To provide support, advice and professional
opinion to the wider multi-disciplinary team on matters within scope of practice.
To demonstrate and apply a broad / high level
of understanding of the effect of disability on mental health and recommend
adaptations to the clients physical and social environment.
To undertake interventions or sessions to provide a sustained programme of therapy or support.
To update and undertake risk assessments and
risk management plans.
Clinical Skills
To work with people with a
diagnosis of mild or moderate mental illness and identifytherapy goals as part of
the overall care plan, Implementing, analysing and interpreting specialisttools to support
recovery and wellbeing.
To manage and prioritise a
caseload.
To plan and implement patient
centred individual or group interventions, using graded activity toachieve therapeutic goals.
To monitor, evaluate and modify
treatment in order to measure progress and ensure effectiveness of intervention.
To apply a good level of
understanding of the effect of illness and provide training and advice on
lifestyle changes and adaptations
to the patients social and physical environment.
To be actively involved in the
discharge process of the patient.
To lead and support the clinical
discussion as part of the MDT.
To be actively involved in the MDT
care planning process alongside DPT and other stakeholders.
Same day service
- To help design and develop the MH and Wellbeing Team contribution to the same day access service providing briefintervention support and signposting to appropriate services.
- To provide timely advice and support on the day, working one day a week supporting the same day service
- Supporting patients to empower them to myselfmanage same day crisis and have appropriate coping mechanisms and be able to access appropriate supportindependently.
- Working as part of an integrated MDT supporting patients with a holsitic approach to care.
