Job summary

Practice Nurse required, to work in a busy GP practice, over two sites.

We will consider part time or full time applicants.

You will manage your own clinics and workload, focusing on chronic disease management for a cohort of patients.

Experience of Primary Care an advantage, but not essential.

Confident working alone and as part of a team.

Main duties of the job

The following are the core responsibilities. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Assessing and carrying out examinations of patients.
  • Develop, implement and embed health promotion and well being programmes
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
  • Request pathology services as necessary

About us

We are a large well established GP practice of almost 13,000 patients. We offer a wide range of services to our patients across 2 sites. Our main site is on Cambridge Avenue in Scunthorpe, our branch site is situated in Messingham.

We currently have 8 GP's, 3 ANPs, a Practice Pharmacist and a well experienced nursing and admin team, all passionate about offering high quality care to all our patients.
