Job summary

We have an exciting opportunity for an experienced Nurse to join our welcoming and supportive nursing team.

The successful candidate will undertake a mixture of general nursing clinics and chronic disease management - ideally you will have respiratory and diabetes long term condition management experience.

The applicant may have a qualification in Diabetes management and comfortable with initiating and/or adjusting insulin doses.

We are keen to hear from practice nurses with any LTC management including respiratory.

To provide specialist care to chronic disease patients within the Practice, working autonomously with these groups of patients, following Practice approved protocols.

Our working day is typically 8.00 am - 6.00 pm, so applicants would need to be flexible to able to start and finish between these times, depending on their choice of hours.

To have a particular interest in diabetes, respiratory or General Practice LTC Management. To implement and evaluate individual treatment plans for patients with diabetes, that promote health and wellbeing.

To work as part of the Practice nurse team providing general practice nursing services to patients.

To assist with the leadership and good management within the nursing team.

Work autonomously following local and national guidance.

KNOWLEDGE, SKILLS, AND EXPERIENCE

Essential - RGN, Diabetes Diploma, Minimum of 1 years post qualification experience essential, Evidence of continued professional development, Excellent communication/Interpersonal sills, Leadership, time management and organisational skills, Computer skills

Desirable -Other chronic disease management experience, General Practice Nurse experience, Nurse Prescriber or willingness to complete, Experience of working in general practice, Experience of using system one clinical system

PRIMARY RESPONSIBILITIES

Support the nurse team to provide a suitable learning environment that is conducive to the acquisition of further skills and knowledge for all staff, for formal and informal personal and professional development.

To work autonomously with patients with LTC chronic disease management following local and national guidance.

Assess, develop, and implement nursing care programmes for patients with diabetes and provide specialist care to these groups of patients.

Teaching and educating patients and their families by offering information, which enables them to understand their illness and its progress.

Select, carry out and interpret results of specific tests, investigations, and act on results e.g., blood tests (HBA1C).

Manage medication treatments as required following Practice protocols.

Provide specialist advice and knowledge to newly diagnosed diabetic patients and their families, including dietary advice, and monitoring of their illness, empowering patients to take control of their illness demonstrating empathy, and giving reassurance and support.

Undertake annual reviews autonomously, commencing/altering treatments as necessary.

Refer patients directly to other specialists in other professions, as appropriate.

Act as a resource, providing specialist knowledge, gained through clinical experience and through undertaking diploma level training and expertise to other members of the multidisciplinary team including GPs, registrar students, medical students, nursing students, district nurse students.

Participate in clinical audits/research projects and implement changes as required.

Participate in the development and updating of Practice protocols/guidelines and procedures within the Practice.

To undertake any aspect of Practice Nursing duties including (but not limited to):

Taking cervical smears/triple swabs and interpreting and acting on swab results.

Travel health advice and vaccination.

Administer injections under individual patient prescription or Patient Group Directives.

Maintain safe storage, rotation and disposal of vaccines and drugs.

Apply infection control measures within the Practice according to local and national guidelines.

Keep up to date with safeguarding policies and procedures.

Deal with complex and sensitive information with patients, relatives, and colleagues.

Recognise psychological needs of patients presenting with depression or suicidal tendencies.

Competent in anaphylaxis and resuscitation procedures.

Promote the health and safety of patients and work colleagues by identifying hazards and assessing clinical risks within the workplace and ensure that all practice policies are adhered to.

Responsible for entering details of clinical consultations accurately on computer records, always maintaining patient confidentiality.

To undertake any other duties which are commensurate with the level of and responsibility of the post.

To always provide an exemplary standard of professional conduct and nursing care in accordance with the NMC.

To maintain personal and professional development, ensuring regular updates are undertaken.
