fundamental nurse-led care to communities across the PCN.
They support public
health and screening programmes including immunisations and vaccination
programmes while participating in quality improvement initiatives to enhance
patient outcomes and reduce health inequalities.

alongside our practices and PCN staff. Our PCN staff currently includes General
Practitioners, clinical pharmacists, physicians associates, social prescribers,
health and wellbeing coach, care coordinators, nurse associates and a paramedic.

The New to General
Practice Nurse undertakes the early detection, prevention, and management of
cardiovascular disease and other long-term conditions. This post-holder is
expected to take a lead role in managing specific long-term conditions
following structured mentorship and preceptorship and work within a
multi-disciplinary team, supporting integrated care and personalised approaches
to patient management.
This role will take
a lead in the following areas:

network and multi-disciplinary team. They are to participate in formal training
events and to promote best practice in their area of expertise.
When a PCN employs
or engages one or more New to General Practice Nurse under the Additional Roles
Reimbursement Scheme, the PCN must ensure that each has the key
responsibilities in delivering health services, as outlined in Annex B (section
B21) of the Network Contract Agreement DES Specification PCN
