An exciting opportunity has arisen to join our multi-disciplinary PCN team.
We are a dynamic, forward thinking PCN
team who are passionate about developing and delivering excellent quality local
services to meet the needs of our patients.
The PCN has been successful in becoming
part of a national pilot scheme which will look at ways to make General
Practice Sustainable. The PCN is mobilising a Home Visiting team to support the
practices with home visits.
We are therefore looking to expand our
team by employing Salaried General Practitioners to work alongside our
practices and PCN staff within the home visiting provision.
Our PCN staff currently includes Clinical
Pharmacists, Physicians Associates, Social Prescribers, Paramedics, Physiotherapists,
Care Coordinators, Nurse Associates and Pharmacist Technicians.

Clinical
responsibilities:
Conduct home visits for patient consultations.
Undertake telephone consultations and in-person
consultations, dealing with queries, issuing acute and authorising repeat
prescriptions, send referrals and correspondences in a timely manner.
Make autonomous decisions regarding patient
issues, assess patients with undifferentiated and undiagnosed conditions, admit
or discharge patients to and from the caseload and refer to other care
providers as appropriate
Screen for disease risk factors, provide
counselling and health education.
Clear documentation of notes.
About us
Rother Valley South PCN is made up of four GP Practices with
a weighted list size of 57,000. Based in South Yorkshire ICB the Network covers
a large suburban area of Rotherham.
The PCN formed in 2019 with no changes to its original
members to date. The PCN incorporated as an LTD Company in 2023 to further
support development within the network.

Clinical
responsibilities:
Conduct home visits for patient consultations.
Undertake telephone consultations and in-person
consultations, dealing with queries, issuing acute and authorising repeat
prescriptions, send referrals and correspondences in a timely manner.
Make autonomous decisions regarding patient
issues, assess patients with undifferentiated and undiagnosed conditions, admit
or discharge patients to and from the caseload and refer to other care
providers as appropriate
Screen for disease risk factors, provide
counselling and health education.
Clear documentation of notes.

guidelines.
Address patient care needs efficiently while
balancing continuity and service access.
Discuss and offer alternative care options when
appropriate and safe to do so.

Report any safety or care quality concerns to
the Clinical Director, Practice Partners, or PCN Manager

Development:
The post-holder will participate in training events organised by RVS Primary Care Network relevant to the role,
as part of the employment
Participation in an annual individual performance review, including taking
responsibility for maintaining a record of own personal and/or professional
development for purposes of annual individual performance review, appraisal and
revalidation.
Participate actively in clinical and practice
meetings.
The PCN and Practices have a strong ethos around
training and development of others. The successful candidate will be willing
and able to support medical students, and other training roles.
Take responsibility for own development, learning and performance and
demonstrate skills and activities to others who are undertaking similar work.
Awareness of and compliance of relevant practice policies/guidelines, e.g.
prescribing, confidentiality, data protection and health and safety
