Job summary
We have an exciting opportunity for an experienced practice nurse to join our friendly team of General Practice Staff with a focus on supporting our patients living with diabetes.
We are looking for a motivated team player who is highly organised, enthusiastic and patient-focused with a can-do attitude and friendly personality. Candidates must have evidence of Diabetes training (Diploma, PitStop or other accredited course)
We can consider candidates seeking 28 hours per week with a working pattern to be agreed.The role allows part time working and has some flexibility in the hours worked and will allow the candidate to maintain an effective work life balance.
This role has the opportunity to develop managerial skills.
Main duties of the job
To work as part of our nursing team to provide a diverse and comprehensive range of nursing services and care to our patients.
To focus on supporting patients living with diabetes to independently manage their condition. To support colleagues with diabetes queries and help to develop the diabetes strategy.

We are a close-knit team of 2 GP partners, 3 salaried GPs, a Nursing team of 4 Registered Nurses, an Advanced Nurse Practitioner and 2 HCAs along with 13 admin and reception staff. Job responsibilities
- To work within clinical boundaries, demonstrating safe clinical decision-making, delivering high quality clinical care.
- To provide care and structured education to patients with diabetes.
- Become the Diabetes Champion and attend meetings and updates as required.
- Participate in reviewing Diabetes protocols when required.
- To provide essential treatment duties such as BP checks, Long Term Condition Reviews, NHS Healthchecks, prescribed injections (e.g. B12, Depo-Provera and Denosumab) BMI and lifestyle advice
- Provide holistic assessment of wounds and implement wound care including complex wound management in line with current evidence-based guidelines
- Offer advice about childhood and adult immunisations and ensure vaccines/injections are administered under patient group directives (PGDs) or individual prescriptions (PSDs)
- Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
- Actively participate in seasonal and special projects as requested e.g. flu campaign
- Act as chaperone when required
- Vaccine / cold chain storage, monitoring and recording
- Undertake health promotion to help educate patients
- To maintain accurate patient records and enter onto the computer using agreed Read Codes
- To collect data and participate in regular Practice audit and research projects
- To contribute to the preparation of any Practice development plans
- To maintain own annual registration with the NMC and adhere to Code of Professional Conduct
- To maintain own continuous professional development with the support of the practice as required
