appropriate health care to patients as part of the new models of care at
surgery level and within our primary care network. The Pharmacist will act
autonomously within their professional scope of practice providing care for
patients face to face/video consultations, telephone consultations and routine
scheduled clinics.

busy rural Practice with around 6000 patients operating in Cinderford in the Forest
of Dean, with a proactive involvement in a successful Primary Care Network.
We have a positive working
atmosphere with regular clinical meetings. We are a dynamic team who
communicate well and adapt to change responsively.
Our PCN comprises 4 GP
Practices, delivering patient centred care to circa 25,000 residents. By
working together in our PCN, each Practice is able to offer a wider range of
services, giving patients faster and more efficient access to the right care
and support.

Clinical Practice
- Work as part of a multi-disciplinary team to deliver safe and high quality care.
- Carry out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
- Delivery of care in line with NSF, NICE guidelines and evidence-based care
- The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all practices of the PCN including working with members of the wider healthcare team to improve care and support safe and effective prescribing.
- The post holder will need to be proactively involved in quality improvement and audit as well as managing some aspects of the Quality and Outcomes Framework.
- There will also be availability for the post holder to contribute to the provision of out of hours/extended hours/improved access services.
- The post holder will be supported to help develop the role and work to improve the care and outcomes for the patients across Forest Green PCN.
- To play a pivotal role in the delivery of high quality primary health care within the Forest Green PCN Making recommendation for/or prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
Care Management
- Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition.
- Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking.
- To monitor and meet care outcomes against standards and recommend or initiate changes as necessary.
- Clinical medication reviews for patients with single or multiple long term conditions where medicines optimisation is required.
- To contribute to the surgery on the day offering if requiring pharmacist input.
- To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission.
- To be involved in multidisciplinary clinical meetings within the PCN where appropriate
- To provide medication reviews for our patient groups in a variety of mediums including face to face, telephone and virtual.
- To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required.
- Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance.
- To maintain full and complete records of all patient contacts using appropriate clinical templates and coding
- To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate.
- Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary.
- To identify areas of clinical risk at network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resources.
- To monitor network practices prescribing against the local health economy and make recommendations for GPs or amend within scope of practice, based on findings.
- To maintain own clinical and professional competence and be responsible for own continuous professional development.
- Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.
Administration and Professional Responsibilities
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
- Participate in the administrative and professional responsibilities of the practice team
- Accurate and legible notes of all consultations and treatments are recorded in the patients notes
- Ensure clinical systems kept up to date, recording and/or amending accurate details
- Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
- Attend and participate in practice and PCN meetings as required
Trainnig and Personal Development
- Maintain up to date skills and knowledge, maintaining awareness of professional issues at an advanced level.
- Training needs will be monitored by yearly appraisal and will be in accordance with the Practices requirements. Personal development will be encouraged and supported by the practice.
- Assess effectiveness of care delivery through self-reflection and peer review, as well as benchmarking and formal evaluation.
- Contribute to the identification and assessment of learning needs of staff and other professionals and assist in planning effective programmes of education.
- Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
- Act as a mentor for junior staff, assessing competence against set standards.
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work and ensure own educational commitment is at least sufficient to maintain revalidation requirements.
- Develop and maintain a Personal Learning Plan.
