Job summary
The post holder is responsible for ensuring the delivery of safe and effective nursing care to the practice population. Leading the nursing team, the post holder is accountable for nursing service delivery. They will lead and manage the nursing resource of Practice Nurses, Health Care Assistants and Phlebotomist, working closely with the practice management team and GPs to deliver the clinical and business priorities.
Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership and direction for the nursing team.
Main duties of the job
To Assess, plan, develop, implement, provide and evaluate treatment programmes for patients presenting with long-term conditions and support health promotion and disease preventative programmes, providing face to face and telephone appointments as appropriate
- Manage the day to day running of key nursing systems and procedures in line with practice policies
- Develop clinical protocols and procedures, review and update as required
- Oversee and/or develop an effective nurse appointments systems with effective holiday/sickness cover
- Routinely monitor and assess nursing team performance against patient access and demand management targets
- To manage the system of Clinical Supervision/Peer Review, organising meetings, agenda and providing minutes
- To participate in the management of the Quality and Outcomes Framework Programme, maximising achievement of quality targets and providing effective patient care
