Job summary

The suitable candidate will be a registered nurse with a non-medical prescribing qualification. They will have an expertise in long term conditions such as Diabetes and Heart Failure.

The post holder will work collaboratively with the whole practice team. The individual will be responsible for ensuring the safe delivery of effective general nursing care to the practice population.

Main duties of the job

The post holder will be responsible for the effective management of patients with long-term conditions, such as Diabetes and Heart Failure. They will also be responsible for all childhood vaccination schedules, along with all immunisation and screening programmes.

About us

We are a forward thinking, supportive team who currently care for approximately 7700 patients with a higher than average elderly population.

Our well-established team includes 7 GPs, 3 ACPs, 3 Practice Nurses, and 1 HCA. We also benefit from the support of the additional roles team within Neston & Willaston PCN, which includes services such as an acute paramedic visiting service, a clinical pharmacy team, and physiotherapy support.

Job responsibilities

Clinical responsibilities:

To assess, manage and treat patients with chronic diseases such as diabetes, heart failure, hypertension and CKD.

To ensure safe and effective care that is evidence based, working within local and national guidelines.

Immunisation & Vaccination Programmes

Proficient in administering the childhood vaccination programme, including unscheduled immunisations.

Seasonal vaccination programmes including influenza and covid.

Routine vaccination programmes including shingles, pneumonia and RSV.

To undertake a thorough travel health assessment to include travel related health risks, dependent on destination and to administer appropriate vaccines.

Womens Health

To be proficient in cervical sampling.Contraceptive and Sexual Health

Wound care and management

To assess patients attending with minor wounds.

To assess complex wounds and undertake doppler assessments.

  • Minimum of five years post qualification experience, with at least 2 years working within General Practice
  • Qualifications in Phlebotomy

  • Smear taker
