Job summary

Neston & Willaston Primary Care Network (PCN) have an exciting opportunity for a motivated Clinical Pharmacist to join our expanding team.

The clinical pharmacy team is integral within the PCN to improve quality of care and patient safety.

The Clinical Pharmacist will be a highly motivated individual who has completed their CPPE pathway and ideally be a non-medical prescriber or working towards this.

Main duties of the job

  • To support the PCN to ensure the safe delivery of repeat prescribing.
  • to support patients and practice staff to optimise medications by promoting good practice inline with clinical and operational guidelines
  • To support the PCN with achieving the prescribing scheme
  • To reduce medication waste

  • To support pharmacy technicians with medicine reconciliations
  • To support all members of the General Practice teams.

    Neston & Willaston PCN was established in 2019 and is one of the smallest PCN's in the country, responsible for approximately 21,000 patients.

    The Network is a collaboration between Neston Surgery, Neston Medical Centre and Willaston Surgery. The PCN work closely within the community, aiming to create a healthier future for our patient population.

    The PCN have a dedicated team who are responsible for planning, sourcing funding and implementing these services.

    The PCN have well-established roles including a Visiting Paramedic Service, Physiotherapy, Social Prescribing Link Workers and a Clinical Pharmacy team.

    Job description

    Job responsibilities

    The Clinical Pharmacy team are integral to the General Practice teams. Their role is to support the general practice teams to improve patient care by optimising treatment.

    The post holder will undertake clinical medication reviews to proactively manage patients with complex polypharmacy. The clinical pharmacist will be responsible for supporting the repeat prescribing process, acute prescription requests and medication reconciliation.

    In order to fulfil this role, the individual will be flexible, be able to work across three sites.

    As the PCN is required to undertake Enhanced Access hours this will also form part of the role.

    The Role

    To undertake clinical medication reviews

    To support patients with their repeat prescribing in line with practice policies

    To be involved with medication reconciliation from hospital discharges and clinical letters

    To be involved with the prescribing scheme as required

    To support all practice staff with medication requests

    To signpost to other healthcare professionals where appropriate

    To undertake prescribing audits as directed by the PCN team

    To action MHRA and other safety alerts as per guidance

    Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved

    To support public health campaigns

    To work collaboratively with all integrated health care teams

    Involves patients in decision making

    Education

    Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council

    Have completed the Primary Care Pharmacy Education Pathway (PCPEP)

    Minimum of 2 years experience as a pharmacist

    Qualified as Non-Medical Prescriber or working toward this qualification

    To ensure knowledge and skills are maintained and all mandatory training is completed

    To work within own competence and local and national guidance

    The to comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.

    To comply with the Data Protection Act (1984) and the Access to Health Records Act (1990)
