We and have an exciting opportunity for an urgent care practitioner to join our well-established and high-performing Practices.
The role will involve working as part of a team with other members of staff (including other urgent care practitioners, nurses, Doctors and a Duty Doctor) to help our patients access acute 'on the day' support they may need, including daily visits to patients' homes. Some routine work when needed.

Main duties of the job
Assess patients presenting with acute illness or for review of acute exacerbations of chronic disease conditions.
Undertake home visits with an elderly care home setting for acute illness.
Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan.
About us
We are a large supportive training practice based in Kingswinford with 20,000 patients and based over two sites. We have a large clinical team consisting of 15 GPs and 25 clinical staff, including nurses and allied health professional all supported by the administrative team.

To provide assessment, diagnosis and treatment at first point of contact by attending to patients autonomously, within the GP surgery, their own homes and residential homes.
To support and continue the development of on the day urgent patient service in the Practice.
To accurately triage and prioritise patients, including in acute situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.
To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To use broad clinical guidelines (NICE etc) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted.
To signpost appropriately to other services in the identified healthcare pathway.
To refer patients to appropriate care settings, within locally agreed healthcare pathways.
To discharge or treat, as appropriate and within the scope of practice.
To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.
To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
To provide clinical advice to other members of the team where appropriate.
