Job summary

Working in accordance with the NMC Code of conduct and with reference to practice agreed protocols and guidelines, the post holder will:

Provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

Provide assessment, treatment, screening, health education service and advice to patients of the practice.

Manage care programmes directly to patients.

Deliver a holistic approach to patient services and initiate and manage a methodical assessment of patient and service needs.

Develop a working environment that promotes health, safe and effective work outcomes.

To have a positive attitude and approach towards helping solve the problems and challenges that face General Practice and be part of the process to help the Practice achieve its goals.

Main duties of the job

Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS health checks.

Document consultations accurately and appropriately, according to NMC guidelines

Ensure awareness of statuary and local clinical adult and children safeguarding protection procedures, including systems of referral. Ability to recognise signs and symptoms of abuse/neglect.

To be responsible for providing the appropriate nursing evidenced based care of an allocated group of patients.

Be an accountable professional who is responsible for their actions, working in line with NMC standards of proficiency.

Recognise and work within the limits of their competency.

To deliver quality care to patients and to work effectively within the nursing team.

To be competent in carrying out a variety of clinical procedures.

About us

We are a friendly Practice of 20 members of staff, we have 6800 list size. Based in Hednesford near Cannock.

Hednesford Medical Practice was formed in 2019 when two surgeries merged Dr Murugan and Dr Chandra. The list size population is as follows. On average this is increasing by 251 patients per year. Dr Sandeep Geeranavar is the GP Partner.

The list size is currently growing on average of 251 patients per year. There have been several new housing estates built over the last 10 years which is seeing an increase of potential patients. The Practice has the capacity to accept new patients.

Hednesford Medical Practice has been established since 2019 and is in its 4th year of service to the public.

Hednesford was a mining village and therefore there is a health impact on residents in respiratory problems such as Asthma and COPD. There is also a current rise in Diabetes within our patients.

Job responsibilities

Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS health checks.

Document consultations accurately and appropriately, according to NMC guidelines

Carry out Chronic Disease Management annual reviews delegated by the Practice Nurse or senior colleague.

Support the Practice Nurse with the management of patients with long-term conditions/chronic diseases, using a holistic patient centred approach.

Identifying significant abnormalities

Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues

Working with patients to develop a management plan where health problems or potential health problems are identified.

Recognising issues and gathering sufficient information to refer (e.g. drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)

Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people)

Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate

Providing in depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.

Ensure competency in anaphylaxis and resuscitation techniques by maintaining and updating CPD.

Support patients and colleagues in adopting sound infection control measures

Apply infection control measures within the practice according to local and national guidelines - This includes:

o Universal hygiene precautions

o The collection and handling of laboratory specimens

o Segregation and disposal of waste materials

o Reporting and treatment of sharps injuries

Dealing with blood and body fluid spillages

Have demonstrated the proficiency to administer medications/immunisations and undertake annual training.

To be aware of and adhere to Practice policies relating to the control, storage and administration if drugs/immunisations.

Recognise and work within the limits of their competence.

Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual and their family.

Support people to improve and maintain their mental, physical, behavioural health and wellbeing, promoting health and preventing ill health.

Develop understanding of all elements of the nursing process and be able to assist the practice nurse in the ongoing assessment, planning, management and evaluation of care.

To recognise changes in the patients condition which require the intervention of others and understand when to refer to the practice nurse and other senior practitioners for reassessment of patient care.

To perform procedures as detailed within the NMC standards for proficiency for Nurse associate (2018) to meet the nursing needs of the patients and their care.

To work within the NMC code of professional practice, and within policies, procedures and guidelines of the Practice.

To have a willingness to undertake additional skills and training dependant on practice requirements.

To ensure high standards of nursing care are given and maintained.

To act and raise concerns when standards are not being maintained following Practice policy and procedures.

To act as an advocate for the patient and the family, including promoting and protecting their interests, privacy and rights, equality and inclusion.

To be involved in PCN initiatives to improve patient safety and care.

Assist in accurate recording of data input to support the QOF.

Liaise with QOF Lead to ensure maximum benefit is gained from the Quality and Outcomes Framework in support of patient care.
