Job summary

We have an exciting opportunity for a Practice Nurse to join our team.

We are looking for a dynamic, committed and experienced individual to work collaboratively with the entire practice team, supporting the delivery of care for the practice population, including patients with acute and chronic conditions.

The successful applicant must have experience of working in general practice and will join a team consisting of a Lead Practice Nurse, Nurses, Health Care Assistants and Phlebotomist.

Main duties of the job

The post holder will work as part of a friendly, vibrant multi-disciplinary team in excellent, modern purpose built premises.

You will be responsible for the delivery of Practice Nursing care to the Practice population. You will be fully supported by our experienced nurses and GP Partners within the Practice and will deliver care within the boundaries of your role, focusing upon supporting patients to be healthy. You will undertake monitoring of long-term conditions, health prevention and screening activities. You will work collaboratively with the wider Practice team working to agreed protocols in order to to meet the needs of our patients.

The candidate must have an RGN degree or equivalent and be registered with the NMC - an interest inRespiratory carewould be an advantage.

Previous Primary Care experience is essential.

About us

Old Mill and Millgates Medical Practice, Poringland, Norfolk is a semi-rural dispensing practice whose patients (some 8,900) are cared for by eight, dedicated GPs in conjunction with a supportive, multidisciplinary team including two Paramedics, a Nurse Practitioner, experienced Practice Nurses and Health Care Assistants as well as excellent Secretarial and Ancillary Staff.

Our aim is to provide the highest standards of clinical care within the resources at our disposal and have an excellent reputation in the local community. Our main branch surgery in Poringland is a modern, state of the art building, large enough to accommodate our attached staff including Midwives, Mental Health Workers, Social Prescribers, First Contact Practitioners, and a Clinical Pharmacist.

We have first-rate training facilities and are a teaching practice currently teaching three years of medical students from the University of East Anglia.
