Job summary

Chorley Central Primary Care Network (PCN) is seeking to recruit an enthusiastic, motivated, and forward-thinking Clinical Pharmacist to join our expanding pharmacy team.

This is a fantastic opportunity to become part of a well-established, dynamic, and supportive network, where pharmacists play an integral role in delivering high-quality, patient-centred care. You will join a collaborative team currently made up of 5 Clinical Pharmacists and 3 Pharmacy Technicians, working closely with GPs, nurses and other members of the multidisciplinary team to improve health outcomes for our local population.

In this role, you will be at the heart of medicines optimisation, promoting safe, evidence-based, and cost-effective prescribing across our practices. You will have the opportunity to develop your own clinical areas of interest, undertake Structured Medication Reviews (SMRs), and support patients with long-term conditions to achieve the best outcomes from their treatments.

Main duties of the job

  • Provide expert clinical support to GPs and practice staff on all aspects of prescribing and medicines management.

  • Support safe and effective repeat prescribing processes.

  • Carry out medicines reconciliation following hospital discharge or transfer of care, ensuring safe and appropriate prescribing in line with local and national guidance.

  • Identify and proactively manage patients at high risk of medication-related problems, including those on high-risk or complex medicines.

  • Undertake Structured Medication Reviews (SMRs) and polypharmacy clinics, making independent prescribing decisions where appropriate.

  • Work collaboratively with community and hospital pharmacy teams to improve patient outcomes, reduce risk, and enhance continuity of care.

  • Provide advice and support to patients through clinics and telephone consultations regarding safe and effective medicines use.

  • Conduct clinical audits, contribute to quality improvement projects, and support achievement of Network targets and practice-based QOF indicators.

  • Assist in developing and implementing local prescribing guidelines, protocols, and pathways across the PCN.

  • Support public health initiatives and provide specialist input into relevant campaigns or health programmes.

    This is a key clinical role within the PCN, focusing on medicines optimisation, long-term condition management, and reducing medication-related harm. Working collaboratively with GPs, nurses, pharmacy technicians, and wider healthcare professionals, the post holder will help ensure patients receive safe, effective, and evidence-based care.

    The post holder will contribute to the achievement of Network Contract DES and Quality and Outcomes Framework (QOF) targets, supporting the PCN to deliver high-quality, patient-centred care in line with NHS priorities.

    Main Duties and Responsibilities Clinical Responsibilities
    • Conduct Structured Medication Reviews (SMRs) for patients with complex polypharmacy, frailty, or long-term conditions, ensuring safe and effective medicines use.

    • Support medicines reconciliation for patients recently discharged from hospital or other care settings.

    • Identify patients at high risk of adverse drug events and take proactive steps to reduce harm.

    • Manage repeat prescribing processes and assist with acute prescription requests.

    • Implement prescribing changes in line with national and local guidelines (NICE, LMMG, ICB).
