Job responsibilities
The post holder is a Pharmacy Technician registered
with the General Pharmacy Council. They work within their profession
boundaries to be an integral part of the Prescribing Team. They are key to the
maximisation of the safe prescribing of medications across the Practice,
ensuring that targets for monitoring common medications are met, and working
with colleagues to enhance existing monitoring systems and therefore ensuring
patient safety.
The pharmacy technician also plays an important role in
efficient prescribing, working to improve repeat prescribing processes.
Working within guidelines and competencies agreed with the Practice, they
also engage directly with patients, helping them to better understand and
benefit from their medications. Patient safety sits at the heart of all these
activities.
To play a key role within our
Prescribing Team by providing leadership and support, as well as working
collaboratively with GPs, other clinical staff, and reception teams.
The role will include:
Coordinating discharge summaries and medication reviews, playing a
central role in repeat prescribing processes, liaising with patients, nursing
homes, carers, hospital and community pharmacies. Responding to medication queries and updating
medical records.
Post holders will need to demonstrate
flexibility and adaptability to working in a dynamic and busy environment

Acting as triage and support for repeat
medicines queries
To undertake medication-related audit
Check Eclipse data on a weekly basis
Support in clinical guidance around specialist
only, unlicensed and not recommended drugs
Review high cost items
Review high quantity dressing requests
Review discharge notifications that have
medication alteration
Supporting prescribing clerks and identifying
training needs
Dealing with out of stock queries
Dealing with medicines related complaints
Where possible dealing with difficult
prescriptions that need an action by the GP. To develop the knowledge of other
healthcare professionals to identify and manage poor concordance
effectively. To carry out basic medication reviews
, concordance reviews and refer to Doctors who will authorise
medication changes. Monitor the primary care DMARDS
service. Liaise with NHS Kent and Medway CCG
Medicines Management Team in respect of the practices prescribing and
budget. Assist the reception team and deal
with patient queries. Assist the clinicians to ensure smooth
and efficient running of clinics. Dealing with MHRA alerts and create a
robust system for review. Lead on EPS and eRD
Communication
and Relationship Skills
Build close and effective relationships within
the Practice and within the NHS Kent and Medway CCG Practices.
Build relationships with the PCN and a range
of prescribers and pharmacy professionals including independent contractors.
Communicate with patients and carers who have
a limited understanding of medicines.
Knowledge, Training and Experience
Registered with the General Pharmaceutical
Council (GPhC) .
Meets the specific qualification and training
requirements as specified by the GPhC criteria to register as a Pharmacy
Technician.
Enrolled in, undertaking or qualified from, an
approved training pathway. For
example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines
Optimisation in Care Homes (MOCH).
Evidence of continuing professional
development.
Minimum of 3 years in a medicines orientated
environment.
Computer literacy knowledge of Microsoft
Word, Excel and Power-point and Clinical systems e.g. Emis
Good understanding of issues affecting primary
care prescribing.
Good understanding of issues affecting
medicines adherence
Good understanding of the common medicines
prescribed for long term conditions in primary care.
Good at practical problem solving.
Ability to work and integrate into GP practice
& associated multidisciplinary teams.
Analytical
and Judgement Skills
Development of best practice to inform and
challenge prescribing behaviours in all prescribers and improve quality and
patient outcomes.
Interpretation of prescribing data and
clinical system patient records.
Ability to assess the patient or carers
understanding of taking medicines.
Able to make assessment of the interventions
required to improve medicines adherence.

Responsibility
for Patient/Client Care
Support the provision of advice on prescribing
to all prescribers.
Support the provision of advice to patients on
medication as required.
Provides advice on medicines concordance
Provides advice and signposting to healthcare
professionals relating to medicines concordance

Supports practice staff in the implementation
of prescribing change.
Supports healthcare professionals in the implementation
of interventions to improve concordance and with agreed changes to
medication.
