Job summary

In recent years the role of General Practice and Community Care has expanded at an exponential rate. Over time societal change, population increases and role expansion has changed the face of General Practice forever. We are seeing ever more complex patient care needs and targets.

We are recruiting to an experienced Paramedic Practitioner to be part of our PCN Hub Team. Paramedic Practitioners are a highly valuable additional to the Primary Care Team. Folkestone, Hythe and Rural PCN wishes to integrate Paramedic Practitioners into our dynamic workforce, this is an exciting opportunity to be part of our highly organised, friendly and progressive team. We already have paramedic practitioners within our team which will offer our successful applicant opportunity for orientation and supervision as required.

The post holder will be required to travel across the PCN locality and therefore will need to hold a driving licence and be in possession of their own car.

Main duties of the job

Personal Specification

Essential

Completion of undergraduate degree/diploma in Paramedicine or equivalent experience. Working at Level 7

Registration with Health and Care Professions Council.

Completion of two-year Consolidation Of learning period as a newly qualified paramedic

Has a further three years experience as a band 6/7 (or equivalent) paramedic

Desirable

Completed or working towards developing level 7 capability in paramedic areas of practice

Independent prescribing Qualification or steps towards this

Further Post Graduate Qualifications or Diplomas

Experience

Essential

Four years or longer post paramedic qualification including 3 or more years at band 6 level

Desirable

Working knowledge of General Practice

Experience of supporting Care Home Residents

Experience of Minor Illness management

Experience of Primary Care IT Systems

Job description

Job responsibilities

To be responsible for the provision of paramedical services, working as part of the primary care networks multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The paramedic practitioner will be required to work autonomously as part of the team, providing same day response to unwell care home residents, and scheduled minor illness clinics (either via telephone or face to face appointments, they may be required to undertake home visits as we develop our home visiting service. The successful applicant will lead regular Care Home rounds, providing proactive clinical care to identified residents. The role provides a range of services such as assessment, differential diagnosis and treatment, referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure.

The following are the core responsibilities of the paramedic practitioner. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. The paramedic practitioner will work within their scope of clinical practice and:

  1. Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health

  2. Provide routine care to patients as required in accordance with clinically based evidence, NICE and the NSF

  3. Examine, assess and diagnose patients and provide clinical care/management as required

  4. Triage patients and provide the necessary treatment during care home/home visits

  5. Collect pathology specimens as required

  6. Maintain accurate clinical records in line with extant legislation

  7. Ensure that read codes are used effectively

  8. Ensure continuity of care, arranging follow-up consultations or reviews as necessary

  9. Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes

  10. Chaperone patients where necessary

  11. Prioritise health issues and intervene appropriately

  12. Act as the lead clinician when dealing with clinical emergencies

  13. Recognise, assess and refer patients presenting with mental health needs

  14. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

  15. Liaise with external services/agencies to ensure that the patient/care home is supported appropriately (vulnerable patients, etc.)

  16. Adhere to the relevant patient group directives and local clinical pathways at all times

  17. Support the clinical team with all safeguarding matters, in accordance with local and national policies

  18. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

  19. Deliver opportunistic health promotion where appropriate

  • Completion of a minimum of 4 years post qualification experience, with at least 3 years at Band 6
  • Working towards developing Level 7 capability in paramedic areas of practice and has completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework.
