Job summary
We have developed a fully interoperable/integrated PCN Hub - this is a truly transformational project which has been fully supported by NHSE/NHSI and Kent and Medway ICB. We are looking for an experienced registered nurse to be at the forefront of clinical delivery. This post is available due to the current post-holder leaving. You will be joining a supported and motivated team of ANP's, Nurses, Paramedics and Care Co-ordinators who provide clinical triage for PCN e-consult, plus deliver minor illness appointments and provide visiting services to our Care Home Residents. Our Minor Illness clinics are held across several of our member practices.
The PCN have utilised considerable resource and expertise to ensure all digital systems are in place to support clinicians and administrative staff to work seamlessly across our member practices.
We are one of the few PCN's across the country that have our own dedicated offices, where the whole team can work together in an friendly office environment.

Main duties of the job
Our Practitioners provide high level clinical triage within our centralised e-hub, this may result in onward referral or discussion with other services.
The post holder will also be part of our centralised minor illness hub service, this may be via telephone consultation or face to face clinics that are held each day - the venue for the clinics are rotated around our member practices. Supervision and support is provided by our G.P. members. plus carry out care home rounds and urgent response for unwell care home residents (this is on a rota basis).
The PCN Hub is still evolving and developing and our clinicians are at the forefront of new initiatives and ideas and are currently developing a proposal for centralise womens health services for our patient population..
About us
Our PCN has a patient population of just under 50,000 across 7 practices. The PCN team is now around 40 staff delivering 12 centralised services, we have our own office base providing an excellent working environment for staff, this office is very close to our Lead Practice. We have a very motivated work-force always striving to provide excellent care for our patient population. All staff are valued and have opportunity to share ideas for future developments. Our Clinical Director is often working from the PCN offices and has an open door policy. We have just incorporated our new Limited Company and will be submitting our CQC application soon.
All staff are encouraged to develop, with study time supported, plus we have a robust PLT programme to support all staff. We are currently developing Clinical Supervision Groups across the whole PCN.
