Healthcare Assistant - Visiting Housebound patients

Job summary
We are looking to recruit a full-time qualified HCA for housebound patients (community), who will be part of the frailty team completing patients LTC reviews and Frailty reviews, working 37.5 hours per week. This role is fixed term for 12 months.
Main duties of the job
Working across both neighbourhood teams the HCA would provide care co-ordination reviewing patients due their reviews and establishing if reviews are appropriate. This role would include the following:
- Establishing if blood tests and basic management can be done by neighbourhood team staff or if a housebound review is needed and complete.
- Trained to provide diabetic foot checks, routine monitoring and observations, physical elements of mental health and dementia reviews and FEV1 measurements and the basic parts of the COPD and asthma reviews, including phlebotomy.
- Provide a holistic review including elements of the comprehensive assessment for frailty and identify where care planning approach may be needed.
- Link closely with the WFHP care coordinators for frailty, Social Prescribers, Lifestyle Advisers, and would arrange remote reviews with diabetic nurses, COPD nurses, GPs and pharmacists as needed.
- Arrange carer assessment and support and link in with social support via Onside teams and VCS organisations.
- Organise and minute weekly MDT meetings.
