Job responsibilities Working under the direction of the PCN Manager and the Clinical Lead for the Pilot and as part of the wider PCN and Pilot teams, the post holder will be responsible for administration of the Liver Surveillance Pilot, as requested by the management team,
Primary key responsibilities.
Utilise population health intelligence and clinical systems to proactively identify and work with patients eligible for the pilot.
Encourage patient participation in the pilot.
Manage data arising from the pilot and maintain Excel spreadsheet used in monthly and quarterly reporting nationally
Develop and embed systems across the network to improve cancer screening uptake in particular Liver Surveillance, liaising with external agencies as appropriate.
Work with member practices in health promotion
Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care;
Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level;
Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals;
Support the coordination and delivery of meetings as required.
Maintaining accurate patient records on EMIS
Signpost patients to support services, voluntary sector and the PCN Health & Wellbeing Team
Secondary Responsibilities In addition to the primary responsibilities, the Cancer Care Co-ordinator may be requested to:
- Participate in other local initiatives to enhance service delivery and patient care.
- Support and participate in shared learning within the PCN.
Raise awareness of cancer Symptoms and provide education to patients.

Effective Communication
- Ensures clear lines of communication and work collaboratively with all relevant health care professionals and agencies.
- Ability to vary the style and level of communication with individuals in order to meet the differing levels of understanding.
- Maintains good communication, liaison and working arrangements within the PCN and other organisations.
Planning and Organisation
- Manages own work and case load and implements programmes of care in line with current evidence, taking action relative to an individual's health and care needs.
Analytics
- Maintains accurate and up to date clinical records in line with PCN policy.
- Participates in providing data collection monitoring and evaluating reports to the PCN Management team in line with local and national requirements.
- Production and administration of patient satisfaction questionnaire for the pilot
Responsibility for Patient Care
- Identifies and manages risks, including assessment of moving and handling risk and understanding the nature of risk as it applies to the safeguarding of vulnerable individuals.
- Preserves confidentiality and be aware of the Data Protection Act, Access to Health record and Consent for Treatment Guidelines.
- Through appraisal and personal development, plan and identify own personal training needs to meet both professional and organisational objectives.
- Undertake any necessary skills training, professional updates and mandatory training as appropriate to the post and as directed.
Policy, Service, Research & Development Responsibility
- Promotes and advocates Equality, Diversity and Inclusion (EDI).
- Proactively makes recommendations to improve the quality of service delivery.
- Maintains an awareness of current trends within the area of practice and within local and national guidance.

Responsibility for Finance, Equipment & Other Resources
- Has no direct responsibility for finance, equipment or other resources but is expected to maintain stock control and observe personal duty of care in relation to equipment used in own work.
Responsibility for Supervision, Leadership & Management
- Promotes effective inter-professional and multi-disciplinary team working with peers, colleagues and staff from other agencies and provides appropriate leadership within the scope of the role.
- Participates in the induction of new staff and contribute to staff orientation programmes when required.
- Allocates work to and support the development of others and supervise, teach, mentor, and assess other staff as required.
Information Resources & Administrative Duties
- The post holder will be required to have good IT skills.
- Knowledge of gathering data from multiple sources and maintaining accurate records
- Ability to produce reports and provide accurate data to NHSE and Cancer Alliance
Any Other Specific Tasks Required
- The post holder may be regularly exposed to a variety of hazards depending on the area of work.
- The post holder must work within the organisations manual handling policy. Physical activity will be required for short periods of time in any span of duty.
- The post holder may be required to support anxious or distressed patients and relatives and may be subject to challenging behaviour.
- Awareness and management of environmental risks when working with individuals.
