Job summary
The post is for
a PCN Clinical Pharmacist as part an ARRS role. The Pharmacist will be employed
by the practice and will work within, and be supported by, a well established Pharmacy Team. They will take responsibility for areas of chronic
disease management within the practice and undertake clinical medication
reviews including patients with complex polypharmacy. They will
provide support to general practice staff with regards to prescription and
medication queries. They will help support the repeat prescribing process, deal
with acute prescription requests, and undertake medicines reconciliation for
the relevant patients following discharge from hospital.
They will provide
expert advice on medicines to patients and the multidisciplinary teams across
our practices. Other responsibilities will include the provision of clinical
leadership on medicines optimisation, quality improvement and the management of
some aspects of the quality and outcomes framework and some enhanced services
across all sites. They will facilitate integration with community
and hospital pharmacy to improve overall patient care. The role is pivotal to improving
the quality of care and operational efficiencies so requires the post holder to
demonstrate motivation and passion to deliver excellent services across our practices.
They will be supported to develop their role to become a non-medical
prescriber as needed. They will be expected to work independently
and also as part of our Pharmacy Team.
