join our vibrant, patient focused and innovative practice who are committed to
the delivery of high-quality, person-centred care.
Main duties of the job
To
provide and maintain a high standard of nursing care to the practice population
with focus in the delivery of evidence-based practice for patients presenting
with long-term conditions and acute problems. This includes screening,
assessment, treatment and education and taking an active role in supporting the
practice to maximise achievement against local and national targets in respect
of the GMS contract and Enhanced Services.
Registered
Nurse with up-to-date NMC Registration and must have worked in a Primary Care
setting with knowledge of current policies, protocols and national targets.

Butetown Medical
Practice is a training practice and consists of over 10,000 patients with a
supportive clinical team consisting of GPs, Primary Care Paramedics, Practice
Nurse and HCA team. The surgery is located close to the city centre of Cardiff
and located in the Cardiff Bay area formerly known as Tiger Bay.
The Practice area
also covers the traditional multi-cultural communities of Butetown and
Grangetown. Butetown Medical Practice is a highly respected practice serving a
diverse population. We are incredibly
passionate about the work we do with homeless, substance misusers and other
vulnerable groups. Although this work
can be challenging it is extremely rewarding, and we are keen to recruit a like-minded
practice nurse.

To provide nursing services to the patients, including treatments, screening, chronic disease management (Diabetes, COPD, Asthma, Hypertension), anticoagulation monitoring, ECGs, undertake wound care management/dressing techniques, venepuncture, child immunisations, travel immunisations and health advice, contraception and HRT reviews. Stock ordering & stock checks.
Main responsibilities
1. Provide nursing treatments to patients in consultation with GPs as appropriate in accordance with agreed protocols
2. Provide general and specific health screening to the practice population in accordance with agreed protocols
3. Provide advice on health care and minor ailments to patients
4. Run chronic disease clinics offered by the practice
5. Liaise with Health Board staff, secondary care staff, community nursing staff and other relevant bodies to ensure an excellent quality of service for patients
6. Record accurate and timely data in patient records
7. Complete admin responsibilities on a timely and accurate basis
8. Work and communicate well with the admin and reception teams
