Job summary
We are looking for an enthusiastic and experienced
Lead Practice Nurse to join our forward thinking GP Practice at Lingwell Croft
Surgery in Middleton, Leeds. We are looking for 24-37 hours per week; we will
consider job shares.

Ideally we would like candidates:
To be qualified in at least 2 LTCs in
particular a diabetes qualification or be prepared to work towards one.
Have a prescribing qualification
To have had leadership experience
We are a partner owned practice and have just one site with a patient
list size of over 15,800.
Main duties of the job
To lead a
nursing team, responsible for ensuring that the practice provides an optimum
service of nursing care relevant to the patient population; working to agreed
protocols.
Main duties and
responsibilities to include the following:
Meeting patients nursing
care needs as identified, or where care has been transferred to the nurse
by a GP. This will usually be in the surgery but occasional home visits
may also be required.
Initiating and maintaining
health screening and promotion clinics for identified areas of the
practice population, recognizing where modification of systems or referral
is necessary.
Managing the nursing care of
patients with chronic disease. Accurate and efficient input of data for
GMS contract purposes is also required.
Providing leadership to the
nursing team.

We are a premises-owning, four-Partner,
training practice with a growing list size, currently around 15,800. We have
seven Salaried GP's, two Advanced Care Practitioners, a nursing team of 8, and a strong close-knit administrative team.
We have a strong reputation as
a teaching and training Practice. We share our passion for Primary Care through
providing placements for sixth form students, teaching all years of
medical/nursing students, allied health professionals and GP Registrars.
We are strongly embedded in our
local community; the PCN Clinical director and PCN leads are all employed at
this practice, driving forward shared services and new ways of working that are
releveant to our patient demographics.
