Job summary
Boston Primary Care Network is looking for an enthusiastic
self-motivated person to be the important link between a GP Surgery and local
care homes. This is an administrative
position and does not have any clinical care.
You will be the main point of contact for care homes, checking in with
the homes on a regular basis, accessing support for residents as needed and
maintaining clinical records.
You will need to have excellent communication skills, both
spoken and written. We are also looking
for people who can build positive relationships with practices, care homes,
wider PCN team and other people/organisations linked to patient care.
Main duties of the job
- To play a role in the delivery of
high-quality primary health care services
- To establish and maintain effective
liaison with stakeholders including health, voluntary, social and education
resources, attending relevant meetings as necessary
- Refer people and/or introduce them to
appropriate organisations in Boston and nationally (where appropriate) e.g.
voluntary, statutory (local authority) and local NHS organisations.
- Support the PCN Manager in providing
KPI reports for submission as requested.
- Be responsible for the organisation,
planning and of own workload to meet set deadlines.
- Following PCN and practice policies
and procedures as appropriate.
- Undertaking Weekly care home check
ins
- Responding to ad hoc requests from
care homes
- Organising relevant practice meetings
and actions
- Building positive relationships with
colleagues and internal/external stakeholders
- Supporting improvement projects
related to care homes

- Has attention to detail, able to work accurately, identifying errors quickly and easily
- Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines
- Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems
- Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience
- An excellent understanding of data protection and confidentiality issues
- Able to arrange suitable meetings with multiple individuals with often conflicting priorities
