Job summary

Boston PCN are looking for an Advanced Nurse Practitioner to develop and lead our new Wellbeing Team. The focus on the team is to work with our population health management data to identify key cohorts of patients that would benefit from a more intensive holistic approach to their healthcare. This role will lead a multi-disciplinary team consisting of Occupational Therapists, Health and Wellbeing Coaches, Social Prescribers and care coordinator. The team will work together but also engage with key stakeholders. We are looking for an experience and highly motivated OT, who wants an opportunity to shape a service to reduce the burden of disease and help people to live well.

The successful candidate will be able to work autonomously, be very patient centred and aim to improve OVERALL population health in line with requirements of the DES (Directed Enhanced Service) contract. You will work across a variety of settings including the community and a patients own home.

The successful candidate will have excellent interpersonal, empathetic and be able to demonstrate excellent communication skills. An ability to work autonomously and manage complex cases will need to be demonstrated at interview. The post holder will need to demonstrate experience of working either in health, social care or other support roles which have direct contact with people, families or carers.

Main duties of the job

The post holder will be employed by the PCN working as part of a multidisciplinary wellbeing team. They will work closely with the practice managers and Clinical leadership team. The objective of the ANP is to deliver and develop excellent patient care and support patients to achieve optimal health outcomes. The ANP will travel across the PCN area to provide patient care either in their home, in a clinic or "hub" environment. The key priorities will be to:

  1. Act as an autonomous practitioner working independently and as part of a multidisciplinary team and in conjunction with other health care professionals to assess, diagnose and treat the conditions of identified patients within primary care initiating direct referrals as appropriate
  2. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence
  3. To provide education and training to other staff and to lead by example
  4. To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice and PCT population
  5. To focus on improving the health of patients with long term conditions as well as general health concerns and suggest quality improvement methodologies and ways to help practices collaborate for the best health outcomes for their patients

Boston Primary Care Network comprises of 6 GP practices, four of which are in Boston with further practices in Swineshead and Kirton. The PCN has a registered population of over 78,000 patients.
