Job summary
We are looking for an experienced and
proactive Practice Nurse to join our established practice team on a 6 month fixed term contract. .
We would be looking for you to work between 15 - 20 hours possibly over 2 days.
The successful
candidate is motivated and committed to providing high quality care, working
collaboratively with the general practice team to meet the needs of our
patients.

Your duties will include:
Running nurse-led clinics
Chronic disease management (e.g. asthma, COPD, diabetes, health promotion and lifestyle advice,Cervical screening, Immunisations and vaccinations, Contraceptive services - Depo, Supporting QOF and Primary Care Standards priorities, Maintaining accurate clinical records, Infection prevention and control

Located in the heart of Wigan town centre, The Dicconson
Group Practice is a well-established GP Surgery serving a diverse patient
population of approximately 9650 people.
You will be joining a welcoming and experienced clinical
team at the Practice.
This is headed up by 4 supportive GP Partners. We have a dedicated team of 3 salaried GPs, 2 Advanced Clinicians, 3 Practice
Nurses, a Nurse Associate and our Healthcare Associate.
They are supported by our fantastic non-clinical team.,
including our friendly reception team, 2 dedicated secretaries and Data Quality
Lead as well as our management Team of 3.
