Job summary

We are seeking an innovative and enthusiastic practice nurse to join our team.

The successful applicant will be experienced in primary care.

Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.

The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with colleagues and general practitioners.

Structured progression is on offer as nurse are encouraged and financially supported to develop clinical interest areas.

Main duties of the job

We require a Practice Nurse to carry out general practice nursing duties to include all aspects of:

  • Health Promotion and Chronic Disease Management Clinics
  • Childhood Immunisations and Travel Vaccinations
  • Cervical Cytology
  • Wound Care

The successful applicant will work in a medium sized friendly, team orientated general practice.

Stone Cross Medical Centre and Clifton Lane Medical Centre, both have a list size of 5000 patients and uses SystmOne clinical system.

We are an innovative practice team who are keen for the successful candidate to make positive improvements within our nursing services. The candidate will be supported by other colleagues and receive a full induction.

Job Title:Nurse

Responsible to:Partners / GPs / Practice Manager

Job Purpose:

To provide a high quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education

Duties and Responsibilities

Clinical

1.

To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems

2.

Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule

3.

Provide travel health advice and administer appropriate vaccinations

4.

Treat patients with minor injuries

5.

Provide directly and assist GPs with contraceptive services

6.

Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs

7.

Act as chaperone when required

8.

Undertake health promotion to help educate patients

9.

Select, carry out and interpret results of specific tests

10.

Undertake anticoagulation clinics to include interpretation of blood tests and dosing

Administration

11.

To maintain accurate patient records and enter onto the computer using agreed Read Codes

12.

Ensure accurate completion of all necessary documentation associated with patient health care

13.

To collect data and participate in regular Practice audit and research projects

14.

To contribute to the preparation of any Practice development plans

General

15.

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

16.

To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc.) are implemented and adhered to

17.

To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

18.

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

19.

Attend and participate in staff and Practice meetings and in-house clinical training sessions

20.

Any other reasonable duties that may be required from time-to-time
