We are looking to recruit a full time practice nurse
We are a large GP practice based in West Derby, Liverpool with a large MDT to help and support you in your role
We have a nursing team that includes a lead nurse , HCA and a Senior Clinical Pharmacist to advise and support within your role
We are a high QOF and Local Quality Contract achiever and wish to continue our success locally and nationally by growing our team
The role is well supported by a Care Navigation and data team plus an experienced management team
Main duties of the job
Duties of the role to include :
Baby clinics to include baby imms
Travel vaccinations
LTC management
QOF management and also Local Quality contract management
Cervical cytology clinics - to include supporting a PCN Sexual Health Clinic
Routine blood and urine sample collections

We are a large , friendly team that includes GPs , Pharmacists , Paramedics , ANPs , Physiotherapists , Social prescribing link workers and Mental Health and Wellbeing coaches
We have supportive huddles daily and weekly meetings to include clinical education , QOF and SEA management
The role is supported by a team of Care Navigators

Primary key responsibilities
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. To ensure all cytology programmes are provided and national guidance adhered too
b. To work as a participating member of the team to ensure nursing participation within all quality contracts ie: QOF/GP Spec
c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
d. To develop, implement and embed health promotion and wellbeing programmes
f. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
g. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
h. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
i. As required, to diagnose and manage acute and chronic conditions
j. To review patient medications to enhance compliance
k. To provide guidance to patients on the use of prescribed and over-the-counter medications regarding side effects and dosages
l. To co-ordinate the provision of travel medicine services
m. To request pathology services and process pathology results as required
n. To participate in the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
o. To maintain accurate clinical records in conjunction with extant legislation
