Job summary

Are you a Practitioner with a specialisation in Elderly Care? We are looking for Band 8 Independent prescriber Advanced Care/Nurse Practitioners to work in our Community & Care (Intermediate Care) Bed Service at Wharfedale Recovery Hub.

The service oversees the care of medically optimised patients who are discharged from hospital following an acute admission but are not yet able to manage/ return to their homes. During their admission to the community bed facilities a multidisciplinary approach to their care focuses on rehabilitation and maximising independence. Our service is responsible for their medical care alongside input from Consultant Geriatricians and allied health professionals including physios, Occupational therapists, SALT and dieticians.

The role involves working closely with GPs onsite and remotely.

Main duties of the job

  • The role will act as an autonomous practitioner working as part of a multidisciplinary team assessing, reviewing, and treating residents in in the community care bed setting,
  • The role is based within Wharfedale Recovery Hub community care settings.
  • They will be responsible for helping to deliver elements of the Community Care Beds contract which includes:

  • Preparing for and attending MDT meetings alongside GP colleagues
  • Personalised Care Planning /Comprehensive Geriatric Assessment for new admissions
  • Liaising with wider MDT involving secondary, community and social care to set goals and plan discharges
  • Reviewing, assessing, and treating patients with new or ongoing medical problems
  • Ward rounds.

About us

Overview of your organisation

We cover 4 intermediate settings across Leeds which are aligned to 2 surgeries, with Wharfedale Recovery Hub and Adel Manor being in the northwest and aligned to Guiseley and Yeadon Medical Centre.

We have several GPs who work within the service and also GP practices. GPs are onsite in the community care bed settings most days of the week and when not are available from their work in the surgeries to give remote support and advise.

There are governance meetings for the service and clinical meetings at the surgery which can be attended for ongoing professional development.

We are keen to support professional development in this role.

Job responsibilities

Clinical Responsibilities:

- The practitioners will work within the multidisciplinary teams made up of nurses, physiotherapists, occupational therapists and social workers.

- They will carry out new resident reviews, including elements of the comprehensive geriatric assessment and advanced care planning,

- Attend weekly MDT at the community care bed facilities and work alongside lead GP, Care of the Elderly Consultants, care home staff and allied health staff.

- Respond to requests for advice and medical review of residents for acute and chronic general practice related problems,

- Liaise with Lead GP for service on daily basis and update them,

- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

- Provide support and advice to patients, relatives, carers, care homes and the wider team in relation to end of life care.

- Proactively identify, diagnose, and manage treatment plans for patients that have minor illness or injury and are at risk of developing a long-term condition (as appropriate)

- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan,

- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice,

- Work with patients in order to support adherence to prescribed treatments,

- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care,

- Meet and discuss with residents, carers and their residents future care plans as needed,

- Manage associated admin from visits including referrals and review of pathology results.

- Report and escalate any safeguarding concerns,

- To work autonomously and is accountable for own professional actions,

- Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to patients at the practice,

- Assess diagnose and treat patients attending with a range of acute, non-acute and chronic medical conditions and refer to appropriate agency.

- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs or with undifferentiated diagnosis,

- Review and action path reports

- Directly admit patients to secondary care hospital in acute medical need

- Clinically examine and assess patients needs from a holistic approach including the physiological and psychological perspective and plan clinical care accordingly.

- Proactively identify diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate,

- Undertake and interpret a range of diagnostic tests and routine clinical procedures e.g., venepuncture and blood pressure monitoring.
