Job summary

Come and join our friendly team at Elmswood Surgery in Sherwood! The role is between 16-20 hours per week. You would be part of our friendly and committed nursing team of 1 Lead Nurse and 2 HCA, and part of our wider team of GPs, pharmacists and physio.

Each of the nurses, supported by the admin and clinical team, lead in particular areas of QOF and/or practice targets. We are a high performing practice, with an excellent reputation for delivering responsive and patient centred care.

You must be reliable, professional and friendly with a can do and flexible attitude to work. We are looking for someone who is an excellent communicator, possesses good IT skills and is able to work independently and as part of a team.

Main duties of the job

The successful applicant will be required to provide the full spectrum of Practice Nurse duties including: Management of long term conditions (Diabetes, Asthma, COPD, Hypertension), wound care, sample taking, ECGs, cervical cytology, ear irrigations, childhood and adult immunisations and travel vaccines. A qualification in management of a long term condition is preferred but not essential.

Work as part of the Nursing team to assess, plan, provide and implement and evaluate nursing care to meet the care needs of individuals in the practice population

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Assist in and perform routine tasks related to patient care as directed by GPs

Working alongside the GPs to manage Chronic Disease including Asthma, COPD, Diabetes, Heart Disease etc

Contraception

Cytology

Wound care /Ulcer care/ Compression

ECGs

Hypertension Management

Routine & Childhood immunisations

Travel Vaccinations

Following clinical protocols

Participation in administrative systems in the Practice

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Work alongside patients to develop individual treatment plans for chronic disease

c. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

d. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF (inclusive of contraception and cytology)

e. Provide wound care (ulcer /Doppler etc.) to patients

f. Provide travel medicine services

g. Undertake services such as NHS Health Checks, ECGs, ear syringing, local enhanced or incentivised schemes

h. Request pathology services as necessary, processing and interpreting results as required

i. Refer patients directly to other services as appropriate

j. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

k. Maintain accurate clinical records in conjunction with extant legislation

l. Ensure relevant clinical codes are used effectively

m. Maintain chronic disease registers

n. Develop, implement and embed well woman/man clinics

o. Chaperone patients where necessary

p. Assist GPs when required

q. Prioritise health issues and intervene appropriately

r. Support the team in dealing with clinical emergencies

s. Recognise, assess and refer patients presenting with mental health needs

t. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways

u. Implement vaccination programmes for adults and children

v. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

w. Attend practice Safeguarding and governance meetings

x. Based on skills, experience, knowledge and gaps in the team, have areas of work with which you lead, where you maintain good levels of knowledge, share this with the rest of the team and support the practice with audits and targets within the lead areas.

y. Support training and provide mentoring where needed of new staff and medical/nursing students

z. Ensure compliance with CQC standards in all areas of work, including keeping records updated in line with CQC expectation

aa. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

bb. Support the clinical team with all safeguarding matters, in accordance with local and national policies

cc. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

dd. Deliver opportunistic health promotion where appropriate

Work with the IPC lead to ensure Infection Prevention Control meets required standards and is effective.
