An opportunity has arisen for an experienced practice nurse to join a modern and innovative clinical team comprising of GPs, Advanced Clinical Practitioners, pharmacists, practice nurses, associate practitioners and HCAs to deliver primary care services in Portslade, Hangleton & Brighton.
We are interested in developing our team of nurses and are keen to encourage applications from people who need/wish to maintain a good work/life balance.

Main duties of the job
Candidates will be able to deliver care in some of the areas listed below and be interested in further training and development.
Cervical Screening. Chronic Disease Management Diabetes/COPD/Asthma/ CHD etc. Complex wound management, Vaccination and Immunisation programmes for both adults and children, Contraception reviews. There will be an opportunity to extend your skill set through training and role development.

Job responsibilities
Successful candidate will perform the following procedures:
- Health promotion and wellbeing, plus
individual plans for patients with/at risk of a long term condition
- Manage and treat long term conditions
in line with national and local policies and practice needs
- Support and manage health needs of
women presenting for family planning and cervical cytology consultations
- Implement and participate in
vaccination and immunisation programmes for both adults and children including
those travelling abroad
- Meet the needs of patients presenting
for opportunistic wound care and those whose wound care needs are more chronic
in nature and require the appropriate specialist knowledge to diagnose and
treat complicated aetiologies
- Diagnose and treat minor illness
within the scope of your professional experience and knowledge, referring where appropriate to colleagues (GPs and
ANPs) if further assistance is required
- Demonstrate sensitive communication
styles to ensure patients are fully informed and consent to treatment
- Communicate effectively with patients
and carers, recognising the need for alternative methods of communication to
overcome different levels of understanding, cultural backgrounds and preferred ways of
communicating, including those receiving bad news
- Participate
in practice team meetings
- Recognise and work within own
competence and professional code of conduct as regulated by the NMC

Produce accurate, contemporaneous and
complete records of patient consultations, consistent with legislation, policies
and procedures
- Work within policies regarding
domestic violence, vulnerable children and adults, substance misuse and
addictive behaviour and refer on to relevant authorities as appropriate
- Take responsibility for own
development, learning and performance including participating in clinical
supervision and acting as a positive role model
- Support the development of others in
order to maximise staff potential and service delivery
- Manage and assess risk within the
areas of responsibility, ensuring adequate measures are in place to protect
staff and patients
- Monitor work areas and practices to
ensure they are safe and free from hazards and conform to health, safety and
security legislation, policies, procedures and guidelines
- Act as a role model to support
members of the nursing team to undertake mandatory and statutory training
requirements
- Apply infection control measures
within the practice according to local and national guidelines
