Job summary

Altrincham Healthcare Alliance Primary Care Network (PCN) is looking to recruit a Independent Prescribing Clinical Pharmacist to join a PCN of five GP Practices covering 48,000 patients in South Trafford. Working 30-32 hours per week, the ideal candidate will be a forward-thinking, enthusiastic individual with initiative who is keen to work within a team of like-minded people and deliver excellent pharmacy-led services. You will become part of an evolving team of 7 Clinical Pharmacists with clinical support from the Lead GP.

Working across the five GP practices within Altrincham Healthcare Alliance PCN, you will work closely with GPs, nurse practitioners, social prescribers and existing pharmacists as part of a multi-disciplinary team while also building strong effective working relationships with the local community, hospitals, the PCN community pharmacy lead and wider pharmacy teams to improve the care and services to our patients.

The role will focus on achieving PCN initiatives as set out in the PCN contract, NHS Long Term Plan and deliverables agreed by the network members to improve place based patient care.

Main duties of the job

Key responsibilities of the role include

Management of patients with chronic diseases to proactively manage patients with complex polypharmacy

Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas

Provision of specialist expertise in the use of medicines whilst helping to address public health and social care needs and tackling inequalities across the PCN area

Development of relationships and close working with pharmacy professionals across the wider health and social care economy

Provision of leadership on person-centred medicines optimisation and quality improvement

Delivery of first-class, professional service to support patients and GP practices across the PCN

Job responsibilities

To support Altrincham Healthcare Alliance Primary Care Network in all aspects of medicines optimisation.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include-

Structured Medication Reviews

Management of long-term conditions

Management of medicines on transfer of care

Reviewing systems for safer prescribing

Addressing both the health and social care needs of patients

Contributing to achievement of QOF and locally commissioned quality improvement schemes.

Undertaking clinical audit

Developing, promoting and implementing the high quality, evidence based and cost effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).

The specifics of the role in the GP practice will be worked up between the post-holder, the PCN and GP Partners.

The post holder will be supported by existing pharmacists employed within the PCN and be supported by the Lead GP. They will also engage with other pharmacy colleagues in Trafford and Greater Manchester for peer support.
