In collaboration with Dementia UK, Mendip PCN are looking to recruit a registered nurse(RN, RNMH, RNLD) with a passion for, and experience of, providing relationship-centred dementia care with a strong sense of advocacy and equalities for this client group to launch a new Admiral Nursing service.
This would be the first Admiral Nursing service in Somerset and an exciting opportunity to make a profound impact on dementia diagnosis, care, and family support across our local communities -and we are looking for a dedicated nurse with a willingness to develop a new service within our Frailty Team & PCN
Main duties of the job
Admiral Nurses are specialist dementia nurses who transform the lives of people living with dementia and those who care for them. In this brand-new role, you will not only provide exceptional, relationship-centred support to families with complex needs, but you will also help build, shape, and influence what outstanding dementia care looks like across Mendip.
Admiral Nurses are specialist dementia nurses who work with and support people with dementia and their families who have complex needs. As an Admiral Nurse you will provide specialist one-to-one support, guidance, and practical solutions for families, alongside supporting best practice and providing role modelling across Mendip PCN.
As the Admiral Nurse Clinical Lead you will be balancing the clinical caseload for the service, alongside setting-up, developing and evaluating the service.You will also influence and lead on the delivery of work streams that improve the experience of care for people living with dementia, and their families from pre-diagnosis to post-bereavement.
About us
Mendip PCN is a group of 5 practices within Central Mendip, Mendip Country Practice, Coleford, Beckington Family Practice, Oakhill Surgery, Grove House Surgery and Park Medical Practice both in Shepton Mallet. Our PCN team at Highfield House was established in May 2022, and consists of the following teams which work collaboratively together supporting the five practices. Teams are Health Connectors, Pharmacy Team, Care Co-ordinators, Frailty Nursing Team and a home visiting Paramedic Team.
You will be employed by Mendip PCN and based across Mendip. You will benefit from fabulous direction and unwavering support from a dedicated and highly committed team who care deeply about improving dementia care across Mendip and will receive ongoing support from Dementia UK to maintain and strengthen your clinical expertise as a specialist nurse within dementia care. You will become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and develop professionally, as informed by the Admiral Nurse Competency Framework.
