Job summary

We are looking for an individual with compassion and integrity to join our friendly Health Connections team. You are likely to have experience in a community development role, whether paid or unpaid, in youth work, adult health and social care, or advice and information services. Health Connectors play an important role within our Primary Care Network multidisciplinary healthcare team.

Main duties of the job

You will coach and motivate patients through multiple sessions to identify their needs, set goals, and achieve personalised health and care plan objectives, as well as facilitating groups offering self-management education and peer support.

About us

The Health Connections Team is part of (Central) Mendip PCN - which consists of 5 practices which have worked together since July 2019 - Mendip Country Practice, Coleford, Beckington Family Practice, Oakhill Surgery, Grove House Surgery and Park Medical Practice both in Shepton Mallet.

The PCN team consists of a Complex Care Hub - Lead Nurse and Care Coordinators, Frailty Team, Pharmacy Team, Home visiting Paramedics, Health Connectors, plus a Clinical Director, PCN Manager, Project Transformation Lead, and IT Transformation Lead.

Job responsibilities

Main responsibilities:Social prescribing: helping people to improve their health, wellbeing and social welfare by connecting them to community services. By increasing peoples active involvement with local diverse communities, personal and community resilience is strengthened - and health and wellbeing inequalities can be reduced.

Deliver Heath Connections and PCN projects within the community, including Talking Cafes and healthy lifestyles education programmes, face-to-face or online as appropriate.

Identify gaps in service provision through speaking to patients and feed back to the Senior Health Connector or Manager.

Provide one-to-one and group health coaching support for registered patients, based on what matters to them, with the aim of: improving peoples levels of activation, empowering people to manage their own health and improve their health outcomes.

Manage and prioritise a caseload of patients. Understand when it is appropriate or necessary to refer people back to, or on to, other health professionals/agencies.

Help promote the Health Connections directory, resources, training, groups and one-to-one support across the locality.

Attend and contribute to team, practice and Primary Care Network meetings and events as required by the service.

Participate in regular supervision, reflective practice and continual learning.

Person Specification

Qualifications

Essential

  • 5 GCSE at A to C or equivalent including English & Maths
  • Full clean UK driving licence.

Desirable

  • Degree or other qualification in relevant area (or equivalent work experience).
  • Training in health/motivational coaching and interviewing.

Experience

Essential

  • Knowledge of the personalised care approach.
  • Excellent communication skills both verbal and non-verbal, 1:1 and with groups, and good written skills.
  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers.
  • Understanding of the needs of people with long term health conditions.
  • Ability to organise, plan and prioritise on own initiative, including when under pressure, to meet deadlines and achieve targets.
  • Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports.

Desirable

  • Knowledge of VCSE and community services in your area.
  • Knowledge of how the NHS works, including primary care, local/national health policy and public health initiatives
